In paragraph 1.4.2, add the following at the end of the paragraph: “Contractor shall be responsible for any pre-abatement activities identified by MDACC for any subsequent ACM discoveries within the project limits (including air handler rooms after the two new air handlers are set in place), if any. Pre-abatement activities are those required to allow MDACC abatement contractors to properly access and abate the asbestos containing material. Indicated new air handler unit placement work will not be completed prior to award of contract and the setting of the new air handlers is to be completed by April 1, 2007.” Addendum 1 January 31, 2007 Page 2 of 5 ADD 1-2 SECTION 00250 OWNER’S SPECIAL CONDITIONS, Article 1.4 SEPARATE CONTRACTS: a. In paragraphs 1.4.3, and 1.4.4 add the following at the end of the paragraph: “Indicated work will not be completed prior to award of contract and the setting of the new air handlers is to be completed by April 1, 2007.” CHANGES TO SPECIFICATIONS ADD 1-3 SECTION 06400 ARCHITECTURAL WOODWORK, Article 3.5 HARDWARE SCHEDULE: a. In paragraph A, delete item number 10 and replace with the following: “10. Cabinet Locks: Best 5L series cylinder locks less interchangeable cores, as manufactured by Best Access Systems division of Stanley Security Solutions, Inc. Exact quantity and locations are currently unknown but are estimated to be 24 Double Door Cabinets in addition to those shown in the documents, including ALL cabinets under sinks and new conference room cabinets. Provide an elbow catch inside one leaf with the BEST lock on the other leaf, in locations as directed by Owner prior to installation. (Keying will be determined, and interchangeable cores will be furnished and installed, by Owner.) Include 24 additional BEST cabinet locks in the Base Bid.” CHANGES TO DRAWINGS ADD 1-4 SHEET D101: a. Delete floor plan general note L and replace with the following: “L. b. Refer to project manual for requirements of the legal offsite disposal of all removed materials.” Add the following to the end of the general note I: “Not all conflicts shown on drawings. Contractor to identify all conflicts and reroute systems, as necessary, to eliminate conflicts and achieve properly functioning systems. c. Add floor plan general note N as follows: “N. ADD 1-5 Contractor is allowed to salvage and reuse handrail and wall guard components if proper profile, color, in good condition and lack damage.” SHEET D102: Addendum 1 January 31, 2007 Page 3 of 5 a. Add the following to the end of plan note 05: “Preference is to use existing cable tray but portions may need to be relocated because existing routing may violate cable distance restrictions. North/South cable tray in west hall to remain. Relocate sections to achieve East/West cable tray from West North/South hallway to new telecom room. Extend from telecom room through waiting room over to far East new North/South corridor. Utilize all remaining cable tray to run North/South in both new North/South clinic corridors.” ADD 1-7 SHEET A101: a. Add floor plan general note P as follows: “P. Provide labor and materials (wall anchors and mounting accessories of appropriate type(s), etc.) to wall-mount miscellaneous Owner-furnished equipment and accessories (such as sharps boxes, soap dispensers, chart racks and other medical equipment) at locations directed by Owner within the Contract Limits. Include installation of 250 Owner-furnished items in the Base Bid, and state on the Bid Form a per-item Unit Price to install additional items furnished by Owner for wall mounting (in excess of 250).” b. Add surface mounted paper towel dispenser ( Georgia Pacific, En-Motion, color to match MDACC standard) and surface mounted soap dispenser (Airkem, Ecolab) to the following indicated rooms: Soiled Room - R9.2130 Treatment Room #4 - R9.2397 Treatment Room #5 - R9.2150 Break Room - R9.2178 W&M 1 - R9.2217 Blood Draw - R9.2369 Exact locations of paper towel and soap dispenser in these indicated rooms are to coordinate in field with MDACC approval. c. Delete floor plan note 41 and replace with the following: “41. d. New millwork and gypsum board construction. See A223.D4. Indicated new electric drinking fountain near Housekeeping Room - R9.2118 is existing to remain. Provide new, wall hung, electric drinking fountain (A.D.A. compliant), Oasis Model PF8AM at opposite side of Hallway R9.2101 in the common wall of Break Room - R9.2178. Provide recessed, gypsum board construction of 3'-0" Addendum 1 January 31, 2007 Page 4 of 5 wide by 2'-0" deep at corner of pipe chase located just North of Break Room R9.2178. ASI to follow with exact dimensions of alcove and wall adjustments. Provide necessary water supply, drain lines and electrical outlet for complete installation. See P201, P400 and A226.A3 for additional information. Millwork base and overhead cabinets in the Breakroom to be reduced accordingly to the minimum amount necessary. ADD 1-8 SHEET A103: a. Delete floor plan note 15 and replace with the following: “15. Mechanical room - refer to Owner’s Special Conditions, Section 00250 article 2.1 Alternates for description for Scope of Work in this room. See electrical for lighting fixtures. Locations of light fixtures to be determined in field with MDACC approval. ADD 1-9 SHEET A222: Delete this sheet and replace with SHEET A222 Interior Elevations dated January 31, 2007. Replacement sheet is available at A&E Products as indicated in the RFP. ADD 1-10 SHEET A223: Delete this sheet and replace with SHEET A223 Interior Elevations dated January 31, 2007. Partition type in detail C1, delete entire note “Field verify that minimum of one layer of 5/8" gypsum board is existing. Provide one new layer...” and replace with the following: “Provide a total of two layers of Type X gypsum board and associated metal framing, from slab to deck above, over the entire length of wall (including columns that occur inside the wall) to maintain fire-rated building separation. This can be achieved either by providing an additional layer of Type X gypsum board over existing gypsum drywall construction, or where necessary by, providing new drywall construction using appropriately sized and spaced framing members and two layers of Type X gypsum board. Field verify existing conditions resulting from abatement process to determine scope of work necessary to provide required finish condition.” ADD 1-13 SHEET A641: a. Delete item 15 from the Specialties/Accessories Schedule and replace with the following: Addendum 1 January 31, 2007 Page 5 of 5 “15. Recessed paper towel dispenser - Paper towel dispenser with motion sensor, Georgia Pacific - model Enmotion, cover color to match MDACC standard. Also provide Bobrick recessed paper towel dispenser and waste receptacle, model B-3900. Contractor to remove cabinet door and paper towel dispenser from Bobrick unit and install the recessed Bobrick unit with Georgia Pacific paper towel dispenser mounted inside the Bobrick unit. ADD 1-14 SHEETS M200, M300, M400, M401, M500, E103, E104, E106: Delete these 8 mechanical and electrical drawings, and replace them with the 8 same-numbered full-size revised sheets dated January 12, 2007. Replacement sheet is available at A&E Products as indicated in the RFP. ADD 1-15 SHEETS P200, P201, P400: Delete these 3 plumbing drawings, and replace them with the 3 same-numbered full-size revised sheets dated January 31, 2007. RFI QUESTION & ANSWER RFI#1 Question: Section 15999-1, System Testing, Adjusting & Balancing, Article 1.2.1 states testing & balancing by contractor as part of the contract. Section 00250, Owners Special Conditions, Article 1.4, separate contracts, Ref 1.4.6 Testing & Balancing. Is TAB by owner or contractor? RFI #1 Response: The Owner is responsible for contracting Testing and Air Balancing services separate from the Contract per Section 00250, Owner’s Special Conditions, paragraph 1.4.6.
