Who fills it in?
The individual(s) within the prospective AEO who are preparing for the authorisation assessment

When is it used?
It is completed after the scoping meeting with the ASA facilitator and as part of evidence gathering in preparation to begin the formal assessment process
It is to be used by the applicant to gauge their organisation's readiness for the full formal assessment as an AEO
