Chapter:-1 ESSENTIAL IDEAS OF SUCCESSFUL ACCIDENT MANAGEMENT (Ref: Accident Management Guide, Sept’89, Issued by Directorate of Safety, Rly.Bd.) 1. All Railwaymen, whether on duty or otherwise, should involve themselves in rescue and relief. 2. There should be utmost speed in rushing medical and other relief to the site of accident. 3. There should be utmost care, consideration and courtesy extended to the passengers involved in the accident. 4. Adequate and swift arrangements should be made for food, drinking water etc. to the affected passengers. 5. Employee responsibilities at the site should be clearly defined. confusion. 6. Quick transmission of information, particularly details of dead and injured, should be ensured. 7. Ensure proper preservation and care of the dead. 8. Ensure security of passengers’ luggage. 9. Timely dissemination of information to passengers of evacuation arrangements to relieve panic and create re-assurance. 10. Ensure proper liaison with Civil Administration and Press. 11. Ensure that clues are preserved and restoration operations are well planned and swiftly executed. >>>>>>>>>><<<<<<<<<< SAFETY HAS NO HOLIDAY. 1 This helps prevent Chapter:-2 DISASTER AND TRAIN ACCIDENTS – AN INTRODUCTION 2.1 SALIENT FEATURES OF THE DISASTER MANAGEMENT ACT, 2005 It is the central legislation on Disaster Management around which all the Disaster Management related activities revolve, since its enactment. It legislates a holistic approach to Disaster Management; from mere responding to disasters to greater attention to prevention and mitigation, capacity building and preparedness. The Disaster Management Plan of the Railways has been prepared by taking relevant provisions of this Act into consideration. Focus of Disaster Management has shifted from being relief and rescue centric to prevention, mitigation, preparedness and capacity building. It has shifted from being departmental to multicultural Endeavour and it has become synergy of national capacity and people’s participation. 2.2 DEFINITION OF A DISASTER ON RAILWAYS: Based on the definition of the Disaster Management Act 2005, Ministry of Railways has adopted the following definition of Railway Disaster: “Railway Disaster is a serious train accident or an untoward event of grave nature, either on the railway premises or arising out of railway activity in that area, due to natural or man – made causes, that may lead to loss of many lives and/or grievous injuries to a large number of people, and/or severe disruption of traffic, necessitating large scale help from other Government/Non-government and private organizations”. Many serious train accidents are also disasters and hence, every Railway staff should be in a position to identify the characteristics of different disaster situations. Hence Board have identified and also approved the proposal to nominate GMs, AGMs or CSOs for declaring an untoward incident as a Railway Disaster in their Zonal levels. ‘Disaster Management’ means a continuous and integrated process of planning, organizing, coordinating and implementing measures which are necessary or expedient for1) 2) 3) 4) 5) 6) 7) 8) Prevention of danger or threat of disaster, Mitigation or reduction of risk of any disaster or its severity or consequences Capacity-building: Preparedness to deal with any disaster : Prompt response to any threatening disaster situation or disaster ; Assessing the severity or magnitude of effects of any disaster : Evacuation, rescue and relief: Rehabilitation and reconstruction: First four points have to be dealt with service department before any accident is taking place and last four points have to be dealt with jointly by all departments in control organization and all officers in the Divisions. Help from Government, non-Government and private organization may be taken. A LITTLE CARE MAKES MISHAPS RARE 2 2.3 DISASTER MANAGEMENT PLAN Disaster Management Plan is a comprehensive document and includes all line of actions to be initiated well in advance of disasters and during disasters and after disaster. This plan has to be actualized during course of time to achieve the goal of prevention, mitigation, preparedness, relief and rescue operation in efficient and effective manner to help the affected people of the area. The plans of the Zonal railway should detail for all types of disasters, and action to be taken for prevention, mitigation and preparedness measures by the railway and also the rescue, relief and restoration systems to be kept in place to meet with them. Organized and systematic plan will result in accurate and speedy response. 2.4 TYPES OF DISASTERS Disaster in the Railway context was traditionally a serious train accident, caused by human/equipment failure, which may affect normal movement of train services with loss of life or property or both. This is now extended to include natural and other man made disasters as well. Railway Board has identified three types of Disasters, these are as follow: (a) Natural Disaster :Natural calamities may also cause serious damage to rolling stock, fixed assets, buildings, etc. leading to disruption of traffic with loss of life/property. Landslide Earth quakes Floods Storm/Cyclones/Tornadoes Earthquakes, floods, Cyclones, Land Slides, Tsunami etc (not related to BSP division) (b) Train Accident related Disaster Collisions (with a huge number of casualties), Train marooned (flash Floods), Derailments at a bridge over a river, and coaches falling down; train washed away in cyclone, derailment of a train carrying explosives or highly inflammable material, tunnel collapse on a train, fire or explosions in trains, And other miscellaneous cases etc. (c) Man made Disasters The following disasters/ accidents may be caused by human/equipment failure, which may affect normal movement of train services with loss of life or property or both. Collisions, with huge number of casualties. Derailments at a bridge over a river and coaches falling down. Derailment of a train carrying explosive/highly inflammable material. Tunnel collapse on a train. Fire or explosion on train. Train marooned (flash floods) due to dam bursting, etc. Train washed away in cyclone, etc. Acts of Terrorism & sabotage, i.e. causing deliberate loss of life and/or damage to property :o Setting fire to a train, Railway installations etc., o bomb blast at Railway Station/Train, o chemical (terrorism) disaster, o Biological and nuclear disaster. o Placing of obstructions on track to cause wide spread disruption to traffic o Tampering with railway fittings to cause accidents. o Situation of riots where large scale loss of life and property is involved envisaged. 3 2.5 LEVELS OF DISASTERS CAUSING INTERRUPTION TO TRAIN SERVICES: Railway accidents can be categorized into different levels: Accidents of a magnitude which can be managed by the concerned divisional authorities. Accidents of a magnitude which may require assistance from neighbouring divisions but can be managed by the Zonal Railway and Disasters of a magnitude in terms of their severity or scale of casualties that require active involvement of multiple agencies of the Central Govt. (Ministry of Railways and other Ministries). An Integrated Operation Centre (IOC) has been set up in the Ministry of Home Affairs (MHA) to handle disaster situation on a 24x7 basis. A Standard Operating Procedure has been prepared for alerts of events of different types and a uniform system has been devised by categorizing each type of alert in stages - Yellow, Orange and Red. A disaster should normally be declared when an accident/ situation is beyond the coping capabilities of Railway, and would require large scale assistance from other agencies. Strike should normally not be considered as a disaster, and should be dealt with separately as per extant rules of the zonal railway. COM is the nominated officer to declare an accident or an extra-ordinary situation as a Disaster in public interest. Such declaration will be issued to all concerned with the approval of General Manager. If an accident or any other extra-ordinary situation is declared as a Disaster, all instructions as contained in this Disaster Management Plan would automatically come into force, and officers and staff of all departments would take action as laid down in this book. All officers, supervisors and staff concerned should be fully conversant with various duties listed therein and carry them out without fail. 5 OBSERVE THE STATION WORKING RULE RIGIDLY 3.3 (a) (b) (c) (d) (e) SYSTEM TO BE FOLLOWED FOR REPORTING OF ACCIDENT TO RAILWAY BOARD (AFTER RECEIVING DETAILS FROM DIVISION) In case of any passenger train accident and whenever there is a total interruption of traffic involving main lines, the matter should be brought to the notice of C.R.B by G.M. (in his absence by A.G.M. or C.O.M.) & D.R.M. (in his absence by A.D.R.M. or Sr. D.O.M.) of respective division., irrespective of fact whether the restoration is likely to be achieved within the threshold interval. CSO/ SSO (Traffic)/ DyCSO (Civil)/ SSO (Elect)/ SSO(S&T) should thereafter inform the nominated officer of Safety Directorate in the Railway Board. At Board’s level, Safety Directorate shall issue the message and take follow-up action. In case of landslide, breaches, OHE breakdown, etc. which result in dislocation of traffic for more than threshold value and also in cases of public agitation, the same shall be reported by HQ Control office to Punctuality Cell of Railway Board and dealt with by the Coaching Directorate. Cases of blockade of train services due to public agitation shall be dealt with by the Security Directorate. In addition, periodic (monthly) statement of accidents in all categories shall be submitted to Railway Board in the prescribed format. In respect of the following accidents, a telephonic advice should be relayed to one of the nominated officers at the Railway Board. (a) All consequential train accidents. (b) Any yard accident having serious repercussions on movement of traffic on through/main line resulting in dislocation of traffic for more than the threshold value as per para 2.11. of accident manual. (c) Landslides. Breaches, OHE break down etc., which result in dislocation of traffic for more than the threshold value as per para 2.11. (d) All train accidents falling in category A to H shall be logged into the “Safety information Management System (SIMS) website at http://sims.railnet.gov.in. all consequential train accidents reportable to Board, indicative accidents reportable to Board as well as other train accidents reportable to HQ including all yard accidents are to be input into the system for Online Accident Reporting. Telephonic information about consequential train accidents and indicative accidents reportable to Board is also to be conveyed to Adviser/Safety and director/Safety-II as at present. They need to be Updated by the concerned authorities. 7 24483530 3.4 A LITTLE CARE MAKES MISHAPS RARE REPORTING OF ACCIDENTS BY DIVISIONS TO ZONAL RAILWAY HEADQUARTERS A telephonic advice should be relayed by Divisional Control to Central Control of Zonal Railway Headquarters immediately after the accident in case of following categories of accidents: (a) All train accidents, (b) Any yard accident having serious repercussions on movement of traffic on through line resulting in dislocation on traffic, for more than the threshold value as per para 2.11 of Accident manual. (c) Landslides, breaches, OHE break down etc., which result in dislocation of traffic, for more than the threshold value as per para 2.11. Note: In addition to this, periodic (Monthly) statement of accidents in all categories shall be Zonal headquarters in prescribed pro-forma. submitted to The CHC/Central Control shall in turn inform all concerned at Headquarters office including GM, PHODs etc in the following order:1. CSO 2. CMD (in case of passenger carrying train accidents) 3. Secretary to GM (for information to GM) 4. AGM 5. COM 6. CPTM 7. CTPM 8. Other department controls in Central control. The respective departmental controls will in turn inform their PHODs, HODs and other officers/supervisors. 9. CPRO 10. Dy.COM/Chg 11. Dy.CSO/Traffic, Dy.CSO/Electrical, Dy.CSO/Mechanical and Dy.CSO/S&T on getting information from the Central Control, the Officers Concerned should be present in the Disaster Management Control Room at HQ for getting further information about the accident. As per the need, the Diasaster Management Control Room shall be manned with Officers for coordinating relief works under the overall supervision of COM. In the same manner, in the Divisional Control Office, CHC/Coaching shall arrange to inform the Divisional Officers in the following order:- 1. Sr.DSO 2. CMS/Sr.MS/MS (in case of passenger carrying trains) 3. DRM 4. ADRM 5. Sr.DOM 6. Other departmental controls in control office. The respective departmental controls will in turn inform their branch officers and other officials/supervisors. >>>>>>>>>><<<<<<<<<< 8 ENSURING SAFETY OF THE CUSTOMER IS MOST SACRED THING Chapter:-4 DISASTER PREPAREDNESS – AVAILABILITY OF RESOURCES 4.1 INTRODUCTION: Disaster preparedness includes all of the activities those are carried out prior to the advance notice of a catastrophe in order to facilitate the use of available resources, relief and rehabilitation in the best possible fashion. It starts at local level, its resources are insufficient and it would branch out at National level and if needed the International level. Government, non-Government and private cooperation is organized to save lives and properties of affected people. Railways are generally self-reliant in carrying out rescue and relief operations as a result of having a well organized set up including ARMEs and ARTs. However, major accidents involving heavy casualties in remote areas or in difficult terrain or under adverse weather conditions are possible to be managed efficiently only by mobilizing non-railway resources. Disaster Management mechanism in Railways can be maintained at a high level of preparedness and efficiency by keeping all resources readily available and in good fettle. Resources imply both Railway and non-railway men and material including medical, personnel, transport, volunteers, police and fire services. Resources available in case of a major accident may be grouped into 4 different units, depending on the time frame within which these can be made available after an accident. These are as follows: 4.1.1 RESOURCE UNIT – I- Railway and non-railway resources available on the train, and at nearby surroundings. (a) On trains carrying Passengers following resources are available: i) First Aid Box available with the Guard. ii) First Aid Box available with Train Superintendent and in the Pantry Car. iii) Portable Telephones, Fire Extinguishers in Brake Van with the Guard. iv) 2 Fire extinguishers each in all A.C. coaches, SLRs, Pantry Cars and Loco. v) Portable Telephones with Driver on locomotive. vi) Walkie-Talkie with Guard and Driver. vii) Escort staff on nominated trains (RPF, C&W, Elect, etc.) viii) Cell Phones/Mobile communications with Passengers. ix) Information collected by Train Superintendent/Travelling Ticket Examiner about Medical Practitioners travelling on the train. x) Information collected by TS/TTE about Railway Officers travelling on the train. xi) Railway Staff travelling on the train either on duty or on leave as passengers. xii) Passengers travelling on the train who volunteer their help for rescue and relief work. xiii) Doctor and para medical staff with emergency medicines and equipment on nominated trains, like Duranto. (b) Non-railway resources available nearby: a. Volunteers from nearby villages and towns. b. Transport facilities available at site or passing through nearby LC Gates. c. Tractors with trolleys from nearby villages both for transport purposes and for lighting up the accident site. d. NGO’s working in the area. e. Private doctors, hospitals, clinics, PMC’s in the vicinity. f. Police and other agencies of State Govt. g. Army or Para-military establishments nearby. h. Coolies and vendors at nearby stations. 9 i. Station staff and local Railway administration should requisition help from non-railway sources before railways own rescue team arrives. j. Such local networks are most effective in rushing assistance immediately, especially with regard to: medical succour, additional manpower, rescue equipment, lighting arrangements, transport services, fire fighting tools etc. (c) Railway resources available nearby: (i) Engineering gangs. (ii) OHE staff and signal staff available. (iii) Other resources such as medical facilities, communication facilities, etc. (d) At adjoining Stations: (i) Staff available at adjoining or nearby stations. (ii) Railway resources as given in respective Divisional DM Plans. (iii) Non-railway resources as given in respective Divisional DM Plans. (iv) Resources should be mobilized to send medical team at short notice as given in the respective Divisional DM Plans. 4.1.2 RESOURCE UNIT – II - Railway resources available at ARME/ART depots and elsewhere within the division. (1) ARMEs, ARTs with 140T crane are stabled at nominated stations. (2) Railway medical and departmental resources. 4.1.3 RESOURCE UNIT – III - Railway resources available at ARME/ART depots and elsewhere on adjoining Zones and Divisions. (i) Location of ARMEs, ARTs with 140T crane based on adjoining Zones/Divisions. (ii) Section wise chart of which ARMEs/ARTs are to be requisitioned from adjoining Zones/Divisions. (iii) Resources of men and material available on adjoining Zones/Divisions are given in their data bank and included in the Zonal Divisional DM Plans of respective Zones/Divisions. (iv) Copies of DM Plans of adjoining divisions should be available with the divisional control office. 4.1.4 RESOURCE UNIT –IV - Non-railway resources available within or outside the division. (i) Non-railway resources available within the division. (ii) Non-railway resources available outside the division. >>>>>>>>>><<<<<<<<<< 10 ALWAYS BE ALERT AND AVOID ACCIDENT Chapter:-5 DISASTER PREPAREDNESS - RESOURCES AVAILABLE IN BILASPUR DIVISION & ADJOINING AREAS 5.1 GEOGRAPHICAL INFORMATION SYSTEM (GIS) Data Bank is available in the GIS which stores in the computer of Disaster Management Room of SECR. We get all the necessary information such as the phone numbers of all stations of SECR including the numbers of Police station, SP, Collector, Hospitals, Blood Banks, Road Cranes, and Airport/Helipad, NGOs etc. to effectively respond to the disaster or any train accident within fraction of a minute by the click of mouse. 5.2 ASSETS AND INFORMATION Camera is provided with the break down in-charges (including that of the break down truck) for taking still photographs of the accident-related evidences. The break down in-charge shall be responsible for the photographs. Hooters have been provided at concerned stations where ART/ARME is located in the division. It shall be sounded at 0000 hrs, 0800 hrs & 1600 hrs every day for testing. The performance of the hooter will be repeated to the control, if defective/ abnormal, for necessary action. Sr.DEE(G) is responsible to ensure stand-by power supply and devise foolproof testing system so that it works in case of emergency without fail. Road maps shall be kept by Engineering department in the control and in the break down truck for taking prompt decision on deputing break down truck to the site. This will also help in reaching to various level crossing gates quickly. These maps should also be available with all area nodal officers. Up-to-date telephone directory containing important telephone numbers (BSNL & Railways), including important civil units such as Police station, fire station medical services, should be available in control. SrDOM shall ensure the same. The list of minimum tools & equipments to be kept in 'C' class ART, Tool Van and Road Mobile Van is to be decided by the respective Sr. DME (O&F) of the division. 4 Quantity mentioned in the above list are mimimum requirement. Divisions may keep more quantity of items or more items than the approved quantity as per their requirement. The Standard list of minimum tools and equipments for ART & 140T BD crane has been approved by the competent authority. >>>>>>>>>><<<<<<<<<< THE TIME TO PREVENT ACCIDENTS IS BEFORE IT HAPPENS 22 Chapter:-7 DISASTER PREPAREDNESS – PREPARING FOR BETTER RESPONSE 7.1 ACCIDENT RELIEF TRAINS (ART) a. b. c. ART is normally stabled on separate sidings having double entry for faster movement in both directions Rescue / Restoration equipments are kept as per Railway Board’s instructions. Breakdown specials are kept with the following officials: Mechanical & Electrical (G) tool vans – SSE/SE(Mech.) Engineering tool vans – SSE/SE(P.Way) Overhead equipments tool vans – SSE/SE/JE(OHE/TRD) d. Crane Supervisor will ensure availability of adequate fuel and water in the crane. e. On getting emergency call, the Crane Supervisor must check and ensure correct marshalling of crane according to site requirement. 7.2 AUTHORITY TO ORDER MOVEMENT OF ARME & ART TO SITE On receipt of information about serious accident involving casualties, ARMEs and ART shall be ordered immediately. ii) This decision would be taken by the PCOR/Mech Officer and CHC/Dy.CHC (Punctuality) on duty and nobody’s authorisation would be required for ordering the same. iii) After sounding of siren the ARME and ART should be run out within the stipulated target time. i) 7.3 1. 2. 3. TURNING OUT OF ART/ARME Immediately after the accident alarm siren/hooter is sounded, all staff earmarked for accident relief train shall report at the nominated place and others at the station. All officials concerned shall report at the ARME if ordered, and proceed with the ARME or to undertake any other duty that may be assigned to them. The Controllers/Loco Foreman/Lobby In-charge/TPC/TLC on duty shall immediately take action to: Arrange for locomotive. Any locomotive available should be utilized, preferably Diesel Loco, in OHE Territory. Call the loco crew and accident train relief staff. Turn out the accident relief train quickly to the site of the accident. Target time for turning out the ARME: In case of double exit siding – 15 minutes In case of single exit siding – 25 Minutes. The time is reckoned from the time of ordering to the time of dispatch. Target time for turning out the Break down trains: During Day – 30 Minutes During Night – 45 Minutes The time is reckoned from the time of ordering to the time of leaving the shed/ART siding. An accident relief train when proceeding to the site of accident will have precedence over all other trains. A breakdown train/ Tower wagon must not be detained for Guard and must leave with the In-charge of Breakdown train. A guard must be quickly arranged later. A MINOR OMISSION MAY CAUSE A DISASTER. 23 7.4 SOUNDING OF HOOTER Accident Manual – 7.02. a) ( Sounding of Hooter):Sounding of hooter for turning out of Relief Train/ARME van will be as follows:Sounding of hooter ARME Relief Train 7.5 Total No. of blast 5 3 Duration of each blast 60 seconds 60 seconds Gap between 2 consecutive blast 10 seconds 10 seconds TARGET TIME FOR THE TURN OUT OF RELIEF TRAIN/ARME AND ART 7.02. C (Accident Manual):The Accident Relief Medical Van must be dispatched to the site of accident within 15 minutes from the base station after sounding the hooter where there is double exit siding and within 25 minutes in case of single exit siding with the first available engine. ii) The Accident Relief Train must be turnout/dispatched from the base station to the site of accident within 30 minutes by day and 45 minutes by night after sounding of hooter. i) 7.6 MOVEMENT OF ARME: ARMEs would be given priorities in the return journey also as they may be required at other locations too. ARME will not wait for any officer. It must start when it is ready to start. For late start of ARME what so ever be the reasons, operating department will be responsible. In case of a passenger train involved in an accident or incident of goods train at Manned/Unmanned level crossing, ARMV must invariably move without ART within the prescribed time. No officer/staff is authorized to order for Amalgamation of ART with ARME. If only one engine is available at station where ARME is stabled then ARME should move first. After arrangement of another Engine ART will move. The nominated staff of ARME must reach in ARME within 15 minutes of sounding of hooter. Refer following letters :(i) Letter No.OPTG/SECR/Policy/407/12 dated 26.06.2012. (ii) Dir.Traffic Trans.(POL)/RB’s letter no.2012/TT-IV/12/6 dated 01.06.2012. In case of accident involving passenger trains, ARME must be ordered immediately on receipt of information of Accident and if not required, should be cancelled subsequently. Rules regarding ordering and dispatch of Relief Train and Accident Relief Medical Equipment are reiterated below:(a) As soon as the information of an accident is received, the train ordering officials considering the type/nature of accident and assistance required will issue orders immediately to the officials concerned for working out the Relief Train and /or Accident Relief Medical Equipment. On receipt of this order, the official concerned must sound the hooter, all concerned officials i.e., principal members, relief trains gang, medical team etc. must get them ready and turnout immediately to accompany the relief train and/or accident relief medical equipment so as to dispatch the train within the target time. No detention should be permitted to medical relief and/or relief train on any account at any point unless under special circumstances and with the permission of Sr.DOM/DOM. (b) In case of serious accidents involving loss of life or serious injuries(i) It is of primary importance to send the medical equipment vans/ the boxes containing medical equipment including water drums/tanks with fresh drinking water immediately to the site of accident without detaining for the rest of the relief train to get ready. (ii) Unless it is known definitely that adequate medical assistance and equipment have already reached the site of accident or will reach quicker from other or nearer sources, the medical van/the boxes containing medical equipment should be dispatched at once and in advance, without waiting to mobilize the complete relief train and staff. BE SAFETY MINDED. 24 (iii) Special arrangements should be made for transport of medical equipment and injured/dead at the site of accident. In such cases the Station Superintendent/Station Master of the station of origin of the train or otherwise nominated should arrange atleasts 30 licensed coolies to go with the relief train considering the seriousness of the accident with the instructions that they should work under the direction of Railway Doctor or Senior most official in charge at site. (iv) In order to ensure an adequate supply of drinking water to the injured and other passengers, at least two 4-6 gallons capacity drums with taps in good working order should be made available with the Station Superintendent/Station Master at the station of origin of the relief train. These drums filled with clean drinking water should be transported along with the relief train if required in addition to the supply of drinking water with ARME. (v) The Station Superintendent/Station Master of the station of origin of the relief train or otherwise nominated should arrange with the Station catering officials/Vendors for preparation and supply as much food and sweetmeat as possible for the passengers whenever required. The vendors should also arrange supply of fruit and milk. If required, the catering officials and vendors with food, sweetmeats and milk etc. should be sent with the relief train or by other means to the site of accident for supply to the affected passengers. (vi) The ARME must be accompanied by a team of medical officer and other assistance from its base station. If any Medical Officer resides at an intermediate station whose attendance is also essential at the site of accident, he should be advised before hand to be in readiness and accompany the ARME unless it is known that he has already reached the site of accident or will reach quicker by other means. Due to unavoidable circumstances, if any medical officer failed to accompany the ARME whose attendance is also essential at the site of accident, action must be taken to send him subsequently by any available means. (vii) Train Examiner along with some carriage and wagon staff and train lighting fitter shall proceed with the ARME to the site of accident. The train lighting staff should ensure before departure of the ARME that the high power lights are in proper working order. The person-in-charge of the C&W department at the base station of ARME shall be responsible to arrange such staff from the nominated gang for relief operation or from the on duty staff at the Station/Yard. The Train Examiner on duty at the base station of ARME on hearing the hooter, shall get in touch with his in-charge and the Station Superintendent/Station Master to inquire, if his attendance is required at the site of accident with the staff mentioned above. (viii) The Accident Relief Medical Equipment and/or Accident Relief Train must be allowed to run to the site of serious accident on top priority. Similar priority should also be given to the train to carry and carrying injured persons from the site of accident. The Section Controller and Deputy Chief Controller on duty of the section concerned shall be responsible to ensure this. On non controlled section or in case of suspension of Control system the Station Superintendent/Station Master on duty at the station(s) concerned must take every action in time for the purpose mentioned above. Whenever the ARME and/or ART are/is ordered, the Station Superintendent/Station Master/Chief Yard Master/Yard Master in charge of the Station/Yard as the case may be, if off duty, shall be called to duty; steps to turn out the said train shall not, however, pend on this account. CHC (M/L), CHC(CIC) & CHC(Chg.) will ensure adherence to the instructions. 7.7 MOCK DRILLS 7.7.1 ACCIDENT MOCK DRILLS FOR ARME/ART:The period between two consecutive turnouts of any Relief Train/ Accident Equipment should not exceed three months. If, therefore, relief train/ accident medical equipment has not been called out in the normal course during a period of three months, a practice drill should immediately be arranged. It is further clarified that whenever there is an accident during the three months period need not be conducted instead the details of preparedness viz. turning of ART/ARME, turning up of doctors/staff, restoration process etc. can be tabulated and taken as a mock drills for the purpose of statistics. In a financial year, quarter ending is on 30th June, 30th Sep, 31st Dec and 31st March. These drills should be ordered by the DRM and conducted under the direct supervision of Sr. DSO or an officer not lower in rank than that of a Senior Scale, nominated by DRM. CONSTANT VIGILENCE IS THE PRICE OF SAFETY 25 In carrying out these drills the following points should be carefully borne in mind:- Turning out of ARMV /ART within the prescribed time. Speed of the specials Assembly of staff within the specified time. Handling of ART, HRDs, HREs and other rescue equipment. Logging of events. Functioning of field telephones and communication network. Functioning of generator sets, lighting equipments. Preparedness of first-aides and availability of medical equipment. Preparedness of commercial department to mobilize adequate manpower. Arrangements regarding the drills should be kept confidential. 7.7.2 DISASTER MANAGEMENT MOCK DRILLS:Each division, with information to HQ, should conduct one full scale Disaster Management exercise once in a year similar to such exercise conducted by armed forces. Every possibility to include State/District/Local Authorities, Armed Forces units and NDRF Battalion pertaining to the area may be ensured while conducting such exercises. A joint report may also be submitted to all concerned highlighting the achievements as well as the shortages noticed during the exercise. 7.7.3 MOCK DRILLS ON DISASTER MANAGEMENT vide according to Accident manual Aug – 2014 para 7.18.1 Guidelines for Mock Drill: a) Disaster Management essentially necessitates a stage of preparedness under all circumstances and the efficacy of the arrangements therein can be assessed only by conducting periodical mock drills. b) c) d) e) f) g) h) i) j) Mock drill is a series of hypothetical accident conditions that create forces to act instantaneously to raise levels of awareness of potential accidents and means to deal with them. Objectives i. To gauge the preparedness, detailed planning and keeping all equipment in good fettle. ii To integrate the operational response and to measure overall performance of the exercise. iii To measure performance in regard to accident restoration. Mock drill shall be conducted as per Para 7.18 at least once in 3 months either during day or night. It is desirable not to repeat the drill again and again at the same time or section. Such drills shall not unduly hamper regular working of trains. It may be ensured that no inconvenience is caused to travelling public. Such drills should not lead tp panic which may result in inconvenience or injury to the public. White absolute secrecy and confidentiality shall be maintained regarding the conduct of mock drill, CSO, GM, AGM, Secy. To GM shall be advised in advance of such mock drill. Except under instructions or orders from HQ, this drill shall be confined only to the respective Divisions and on this account, no traffic shall be refused from the adjoining division/Railways. Mock drill trials may end with the departure of ARME/ART. However, if considered necessary, DRM may permit the ARME/ART to proceed upto the mock accident site. In such cases, the drill shall end with the various agencies having set up their field establishment and deployed their equipment. During these trials, the following aspects shall be closely watched by the officer in-charge of the drill. i) Turning out of ARME/ART within the prescribed time. ii) Speed of the specials. iii) Attendance of staff. 26 k) 7.7.4 iv) Handling of accident relief Cranes, HRDs, HREs and other rescue equipment. v) Logging of events. vi) Functioning of field telephones and communication network. vii) Functioning of generator sets, lighting equipment etc. viii) Preparedness of First-Aiders and availability of medical equipment. On completion of the drill, a detailed report shall be submitted within 3 days to the headquarters detailing: i. Response time of ARTs/ARMEs ii. Alertness and skill of the staff. iii Deficiencies noticed and corrective measures initiated if any. iv. Assistance required. MAINTENANCE, OPERATION AND MOCK DRILLS OF 140 TON CRANES. As per Railway Board’s letter No. 2010/Safety (BM)/6/23 dated 23.08.2010 and EDME/Tr. letter No. 96/M (M&P)/175/3/A/T dated 20.08.10 that advising the Mechanical department of all zonal railway to examine the system for maintenance and upkeep of 140 tone cranes and to hold Mock drills (either once in six months/one year). >>>>>>>>>><<<<<<<<<< MAKE SAFE WORKING A REGULAR HABIT. 27 Chapter:-8 DISASTER MANAGEMENT - OUTSIDE RAILWAY AGENCIES 8.1.1 NATIONAL DISASTER RESPONSE FORCE (NDRF) 8.1.2 LOCATION, CONSTITUTION AND FUNCTIONS These have been formed under the Disaster Management Act at 8 selected locations in the country for dealing with relief and rescue operations related to all types of disasters. 8 battalions, 2 each from CRPF, CISF, ITBP and BSF have been placed in different parts of the country. DG, Civil Defence under MHA in additionally looking after the work of NDRF at the center. Out of these 8 battalions, 4 (1 each from 4 ParaMilitary Forces) to deal with Nuclear, Chemical and Biological (NBC) Disasters and balance 4 will deal with Non-NBC Disasters. Each Battalion has 6 Companies comprising of 3 teams each. Team comprises of 45 men out of which 24 are for Search and rescue and balance 21 for support functions. Short-listed & trained staffs are on deputation in NDRF. As per the Disaster Management Act, various Ministries and departments under Government of India should join hands for mutual assistance in case of a disaster. Assistance from local government and NonGovernment agencies is invariably required by the railway administration for prompt relief and rescue operation in case of disaster affecting railways and, therefore, island away from the civil areas (of the Districts/States). In most cases of a disaster, other than a train accident, the State governments as well as the zonal railways would, therefore, requisition the NDRF simultaneously (for the same disaster). 8.1.3 COORDINATION WITH NDRF As per the Disaster Management Act, various Ministries and departments under Government of India should join hands for mutual assistance in case of a disaster. Assistance from local government and NonGovernment agencies is invariably required by the railway administration for prompt relief and rescue operation in case of disaster affecting railways and, therefore, assistance of NDRF could be of great help to the railways. The rail infrastructure is not in an island away from the civil areas (of the Districts/States). In most cases of a disaster, other than a train accident, the State governments as well as the zonal railways would, therefore, requisition the NDRF simultaneously (for the same disaster). Coordination amongst the affected agencies (many departments of the Central Government and the States) is very important before the help of NDRF is required. Zonal Railways should get in touch with NDRF offices at the nearby locations to have the first hand knowledge of the resource available with them and also to familiarize them with railway related disaster situations and expose them to the issues relevant to the rescue and relief of passengers during railway accident. It has also been advised to associate NDRF in full scale exercise that is held once every year. There are no charges for availing the services of NDRF except the rail transportation which railways may provide at their cost for attending to rail disasters. Railways may also have to provide rail transportation logistics for transporting NDRF even in case of non-railway exigencies. The Railway Board had earlier empowered DRMs to directly requisition the relevant NDRF battalion for relief and rescue operations depending on the gravity of situation so that their services could be made available expeditiously without any loss of time. However in the cabinet meeting held on 22nd October 2009, the National Policy on the Disaster Management has been approved. This Policy lays down modalities for the requisitioning of NDRF as under:The general superintendence, direction and control of this force shall be vested in and exercised by the NDMA and the command and supervision of the force shall vest in an officer to be appointed by the Central Government as the Director General of Civil Defence and National Disaster Response Force. THE BEST SAFETY DEVICE IS A CAREFUL MAN 28 8.1.4 REQUISITIONING OF NDRF FOR RELIEF AND RESCUE OPERATIONS IN CASE OF SERIOUS RAILWAY ACCIDENTS. Board’s policy directions given to the Zonal Railways that the DRMs can directly approach for requisitioning the services of the NDRF now stand revised. The need for the services of the NDRF shall be communicated in a centralized manner by the Zonal Railways through the NDMA only. CSO of the concerned railway shall requisition the NDRF in accordance with the letter No. 50/455 / Disaster Management / Pt.V dated 18.12.13 issued by CSO/SER based upon the following JPO . Sub: Requisitioning in the help of NDRF for relief and rescue operations in case of serious Railway accidents. Ref : Discusssions held between Railway and NDRF during the meeting held at Kharagpur on 26.09.2013. During the above meeting one of the important points which was brought out was to streamline the format/procedure for seeking the help of NDRF in case of serious Railway accidents. Primary prevention is to reduce–avert-avoid the risk of event occurring, by getting rid of hazard/vulnerability. Secondary prevention means reorganize properly the event and to reduce its effects. Prevention is concerned with policies and programmes to prevent recurrence of disaster and covers long term aspect of such disaster. Mitigation means to reduce severity of human and material damage caused by Disaster. 9.1.2 a) b) c) d) e) 9.1.3 OBJECTS OF MITIGATION Save Lives Reduce economic disruption, Decrease vulnerability/increase capacity Decrease chance/level of conflict, Matching increase in maintenance support system (both manpower and equipments). VISION – 2020 Stipulates targets Zero accidents and target to achieve Zero failure in equipments and a leapfrogging in technology and to generate committed work force to meet future challenges. Railway Board has prepared Corporate Safety Action Plan and on that basis SECR has prepared Corporate Zonal Safety Action Plan (2003-2013) and BSP division has prepared Divisional safety action plan. For each year Safety Action Plan is prepared to implement the object of Corporate Safety Action Plan to prevent any unusual incidents and to mitigate human sufferings. 9.1.4 SAFETY ACTION PLAN TO PREVENT AND MITIGATE DISASTER. 1. Replacement of over-aged and redundant assets are being taken care of by Railway. Rehabilitation/rebuilding of bridges on the basis of technical obsolescence will be taken up in phased manner. Over aged rails, turn-outs, ballasts, wooden sleepers, replacement of ST&CST by PSC sleepers, through weld renewals, CTR, TRR, TSR etc. are done on regular basis. Maintenance of track and its monitoring are done intensively. Vulnerable areas are inspected frequently and their patrolling is done on seasonal basis and as and when required. Sabotage prone areas are patrolled in emergency and anti-sabotage measures are taken for protection of tracks. 2. 3. 4. 5. 6. Over- aged locos, coaches and wagons are replaced by Railway in time phased manner. Four wheeler wagons are phased out from system. Zero defects and Zero missing safety fittings are ensured at the time of turning out of rolling stock from workshop, loco shed and sick line. 100% Brake powers are ensured on air brake trains from originating station/yard. Guide line on over due maintenance of rolling stocks are strictly followed. Over due lever frames, signal gears are replaced by panel interlocking. Track circuiting, provision of BPAC, Data-logger, LED based signal light and interlocking of L.C gates are being provided through Safety Action Plan. Manning of unmanned level crossing having heavy road/rail traffic, construction of ROB/RUB and limited height subways are planned with coordination with state government. Some unwanted unmanned and manned level crossings are to be eliminated. Basic infrastructures are provided at manned and unmanned level crossing gates. Technological inputs are given priority in maintenance of tracks, rolling stocks, signals and telecommunications and IT. Instruments/Devices like USFD, GPS, VCD, Thermit welding, twin beam head 31 light for locomotives, fire retardant materials, provision of emergency exit, auto flasher light and provision of micro processor- based speed recorder and electronic brake system etc. are guarantying safety in system. Addition of technical inputs will be carried in the system of assets maintenance. Equipments like TAWD, ACD, TPWS are to be planned to plug the human failure and to ensure safety in due course of time. 7. Long-hours duty, sobriety test on board and filling up the vacancies in safety cadres are monitored on regular basis. Ten hours rule will be implemented with all sincerity. 8. Human resource development is managed through 1) formal training in training schools 2) on job training in workshops, loco sheds and maintenance depots.3) Safety seminars are organized to instill safety awareness on different topics. Front line staff and supervisors are imparted on job training on newer technology and equipments. Training on Disaster Management is given to officers and supervisors in different institutions to meet any emergency. Focus on development of man power through major improvements in working environment and training will be given priority. 9. Inspections and Counseling are being conducted on regular basis by all the departmental officers and supervisors and follow up action on field inspections are also monitored properly. Safety-Audit Inspections are done on inter Railway basis of critical Railway establishment. 10. Alert-Advices are issued from time to time and Safety Drives are conducted against the weaknesses of the system for rectification. 11. Accidents are taken seriously and all accidents are enquired into and remedial measures are taken to avert the recurrence. D&A action are initiated against the culprit. GM & AGM/DRM are accepting the findings and review of all accidents enquiry reports. Yard accident reports are being accepted by Sr.DSO. 12. Coordination with other Government agencies for promoting safety at level crossing is maintained. Media campaigns are launched from time to time. 13. In case of any Disaster, Government, Non-Government organization and private agencies are mobilized to meet the emergency jointly to save life and property of people involved. Civil and Private Hospitals are also tied up with railway medical department to get medical assistance in time to save the precious life of injured passengers. Funds required for medical assistance are met with SOP on Disaster Management. Ultimate motto is to reduce the humans suffering and material damage, which is always given paramount importance before and after the accidents. Reducing the vulnerability and to increase the capacity is always kept in mind while dealing with prevention and mitigation of Disasters. >>>>>>>>>><<<<<<<<<< SAFETY IS HOW SAFE YOU ARE 32 Chapter:-10 CAPACITY BUILDING TO HANDLE DISASTER 10.1 NEW CONCEPTS Newer concepts like integration of disaster management into developmental planning, leveraging on the strengths of other non-railway agencies etc. Training on disaster management of various tiers of Railway Officials was envisaged. There was no training given for natural calamities or terrorism related items. Hence concept of training in railway re-oriented to cover new concepts. It has also not yet been possible to harness availability and strengths of railway on-board staff who are the “first railway responders” during a serious train accident. With this in view, board have decided to re-map the training on disaster management being imparted to several tiers of railway officials through railway training institutes as indicated below – Special training modules are being setup at ZRTI and STS for disaster management training of other railway officials. For the rescue, extrication and other essential aspects of fire fighting, crane operation etc, Training is proposed at Bangalore. 10.2 LIST OF TRAINING CENTERS IN SEC RAILWAY:There are Seven Number of Recognized Training Centers and One non-Recognized Training Center are available in SEC Railway to train the staff of all Departments to promptly and effectively respond in Disaster or major train accident. GMs, PHODs, DRMs and other SAG Officers in SEC Railway are taking Disaster Management Training in IRITM/LKO to promptly and effectively respond in Disaster. As per Railway Board’s letter No.E(MPP)2009/3/1 dated 20.01.09 that since adequate number of staff has not been trained in ZRTIs it should be ensured that Disaster Management module should also be given in STCs and other training institute for Railway front line supervisors and staff. Accordingly, Safety Department of SECR has started to provide Disaster Management Training to front line supervisors and staff in Multidisciplinary Training Centre (MDTC) from 20.09.10. >>>>>>>>>><<<<<<<<<< REFRESH YOUR KNOWLEDGE OF RULES FREQUENTLY 33 Chapter:-11 GOLDEN HOUR 11.1 GOLDEN HOUR PRINCIPLE In the period immediately after the accident where grievous injuries to passengers, loss of property etc. takes place, action has to be taken on war footing by Railway Officials/officers on-board to render definite medical care which gives relief to affected persons and also helps them to overcome the trauma. This first one hour period is known as the Golden Hour. During this Golden Hour period, following efforts should be made by the Train Guard, Loco Pilot, Conductors, TTEs and Officers/Staff traveling by Train- i) ii) iii) iv) v) Inform immediately to adjacent stations for local help to injured. Render definite medical care within Golden Hour. Stop bleeding and restore blood pressure. Person under shock shall be immediately relieved of shock. Transport the injured to the nearest hospital. Nominated trained crack teams in Zone and Divisions of rail rescue experts and containerized equipment, to be rushed to any major accident site by air or land route. 11.2 IMPLEMENTATION OF GOLDEN HOUR CONCEPT Advisor, Traffic Trans. (M) letter No. 2004/M(L)/466/710 dated 29.06.2010 a) Most of the self propelled units are located at places where availability of diesel locomotives is not a problem. On the other hand some of the conventional ARMVs and ARTs are so located that availability of diesel locomotive for their haulage may get delayed. On an over all perspective, where diesel loco’s are not readily available, the SP ARMV and SPART need to be based on priority at these locations. This is presently not so. This deficiency needs to be removed by relocating the SPARMVs. b) In order to make available two M/L locomotives in the interim (once loco hauled ARMV and the one for ART) and eventually only one (for ART), the Zonal Railways may review the deployment of locos for making use of available passenger service link and goods lie over diesel locomotives at the same location to have round the clock coverage. Locos being used for local shunting can be used for the ARMV/ART. Replacement of WDS4/WDS6 with a M/L diesel loco for shunting, running of few short lead passenger trains on diesel under OHE, etc. should be done on the basis of a periodical review. c) All divisions have, at present, a few dual traction trained crews. Their usage on dual traction (especially on Diesel loco) will need to be continued even on a selective basis. They may be deployed where possible by using them periodically on Diesel traction to ensure learning road is repeated after every 15/30 days due to non-usage of crew. >>>>>>>>>><<<<<<<<<< A SHORT CUT IS OFTEN A WRONG CUT. 34 Chapter:-12 DISASTER MANAGEMENT CELL 12.1 DISASTER MANAGEMENT CELL The disaster management cell shall be operatesd through a nominated Disaster Management Room, which shall be provided with such communication and media facilities as required for its operation. The cell will be attended by nominated officers of various departments as below:- 12.2 DEPARTMENTS ATTENDING THE DISASTER MANAGEMENT CELL 12.2.1 SAFETY DEPARTMENT The Officer representing Safety Department shall coordinate the functioning of Disaster Management Cell. He shall report the accident to DRM, CSO and update the restoration details from time to time in turn DRM CSO will inform to GM/SECR, CRS and Railway board. He shall also ensure ordering of Relief Train for evacuation of involved passengers from the site of accident. The officers attending the Disaster Management Cell shall obtain the full details of the accident from Divisional Control/Site of accident and shall monitor the movement of Breakdown Train and Relief Train. Top-most priority is to be given for rescue operations, for transferring the injured passengers from the site of accident to the nearest hospitals, and for sending medical aid from Civil/Military/private hospitals. 12.2.2 MEDICAL DEPARTMENT The Officer representing Medical Department shall obtain all details regarding casualties/injuries to passengers involved in the accident and shall maintain liaison with the Accident Site/Referral Hospitals and Dispensaries, and shall consolidate the list of injured/casualties in minimum time so that the same can be faxed to concerned station for display in Emergency Control and Emergency Information Booths. 12.2.3 COMMERCIAL DEPARTMENT The Officer representing Commercial Department shall ensure ordering of refreshments for the passengers of involved train, payment of ex-gratia to the injured and to the next-of-kin of the dead passengers, arrange for trans- shipment of goods and passengers luggage, and hire private buses for transport of passengers from site of accident to the nearest Rail Head. He shall also ensure transmission of detailed information regarding injured/casualties to originating/destination stations, to Railway Board, and to HQ of the originating/destination Railways. He shall also ensure opening of Emergency Information Booths at important junction stations en-route within the Railways. 12.2.4 OPERATING DEPARTMENT The Officer representing Operating Department shall manage relief and restoration operations. He shall record all events related to the accident chronologically. He shall ensure regulation and diversions of trains keeping in view the likely time of restoration at accident site. He shall ensure that the passenger carrying trains are regulated at such stations where water and catering facilities are available. He shall also monitor the ARTs/ARMVs/Labour Special trains ordered from the adjoining divisions and zones. SAFE RUNNING OF TRAINS WILL CONTINUE TO DEPEND UPON HUMAN VIGILENCE. 35 12.2.5 12.2.6 MECHANICAL DEPARTMENT The Officer representing Mechanical Department shall assess the requirement of additional Medical Vans/Breakdown Trains and shall liaison with adjoining Railway/Division for ordering the same. He shall also monitor the movement of Breakdown trains. He shall obtain the details of rolling stock involved in the accident and its PRO particulars. He shall obtain the restoration details regarding rerailment /toppling of wagons/coaches done by each individual Breakdown Train. He shall obtain the bio-data of the crew involved in the accident. ENGINEERING DEPARTMENT The Officer representing Engineering Department shall obtain the information regarding damage to track and shall ensure ordering of material train if required. The details of track structure and other relevant details such as USFD particulars, last inspection, profile of the track, etc. shall be obtained within minimum time. He shall organize ordering of additional Labour and material Specials, as per the requirement at site, from the adjoining divisions/zones. 12.2.7 ELECTRICAL DEPARTMENT The Officer representing Electrical Department shall obtain the details of extent of damage to OHE/LOCO. He shall obtain the bio-data of crew involved in the accident. He shall assess the requirement of additional material at the site and shall organize movement of Tower Wagon/Material-Spl. to the site of accident. Electrical/General department will provide adequate illumination during night time. 12.2.8 S&T DEPARTMENT The Officer representing S&T Department shall ensure efficient communication. A line should be dedicated for the emergency transmission from site of accident/divisional control to Disaster Management Cell. He shall ensure installation of MTNL/Telecom Department Public Phone/Railway Phone at the site of accident involving passenger train. He shall obtain the details of S&T gears involved, if any, in the accident. >>>>>>>>>><<<<<<<<<< A LITTLE MORE CARE ON THE APPROACH OF UNMANNED LEVEL CROSSINGS IS A LOT MORE SAFETY TO THE ROAD USERS. 36 Chapter:-13 PASSENGER CARE 13.1 GENERAL Providing assistance to passengers and their relatives/dependents is of utmost importance in helping them relieve their misery. Injured passengers and their relatives/dependents are to be treated with utmost courtesy and sympathy so as to alleviate their trauma and discomfort. For dealing with relatives/dependents arriving from far-flung corners of the country, staff fluent in local language of the place from where the train originated, should be used as interpreters. Commercial Supervisors and Welfare Inspectors should talk to injured passengers and ascertain if they wish to call their relatives/dependents. Injured passengers should thereafter be provided with either mobile or STD phones to enable them to speak to their relatives/dependents. Transshipment of unaffected passengers and their clearance from the accident site would be arranged quickly. The officer available in the control shall arrange the transshipment on priority. Even in case of worst possible accident with adverse and extreme circumstances, all injured passengers would be rescued on priority. Site manager as well as officer in charge in the control shall monitor the situation to do so. In rescue operations, top priority will be given to all passengers in critical condition for immediate medical attention. Even in case of worst possible accident, dead bodies would be extricated at the fastest possible speed. The ARME in charge and officials available at the site of accident should act accordingly. 13.2 HOSPITALIZATION OF THE INJURED General policy, in case of railway accidents involving passengers, is that of rapid evacuation of the victims to railway hospital after rendering immediate and necessary first-aid treatment. In case there are no railway hospitals nearby, they are to be admitted in the nearest Government hospitals. In the following cases, the injured may be taken to a private hospital: When there is no railway or Government hospital available within a radius of say 8 kms. From the site of accident, or When the attending doctor certifies in writing that the treatment in private hospital is necessary in the interest of the patient. Except where railway doctor certifies, such injured passenger should normally be eligible to the class of accommodation in the private hospitals where different scales are available. Where the family of the injured person desires to be provided with a higher-class accommodation, the family should give in writing to pay the extra cost involved directly to hospital authorities. For this purpose, each division should chalk out a working arrangement with such private hospitals as may be necessary in areas served by them, so that in an emergency, injury cases can be referred to hospitals concerned without loss of time. To facilitate matters and to avoid misunderstanding, CMD should draw a list of such private hospitals bearing in mind the Railway and other Government hospitals in the vicinity. CMD should also fix the charges to be paid in such cases for each class of accommodation. Complete medical care will be taken of all injured passengers, including payment of medical bills till their final discharge from hospitals. Claims compensation booklets containing forms and other instructions will be distributed to all injured passengers and next of kin of all deceased passengers. Bills by such private hospitals should be submitted through CMD, who will certify the correctness of charges payable, before forwarding for payment to FA&CAO. ALWAYS BE ALERT AND ELIMINATE HUMAN ERROR 37 Under this Para, payment to private hospitals can be arranged locally by the Railway, and Ministry of Railway’s approval is not necessary. If the injured are admitted in non-Railway hospitals, railway doctors should be deputed to these hospitals to render necessary assistance, including supplying the medicines that are not available in these hospitals. They should carefully monitor the condition of injured and maintain an updated list with all details. If more than one hospital is involved, apart from deputing doctors to individual hospitals, a railway doctor should also be deputed to co-ordinate and maintains the centralized updated position. 13.3 FACILITIES TO BE MADE AVAILABLE IN THE HOSPITAL There should be a separate reception counter manned by a commercial Supervisor or by a Welfare Inspector at the entry to the hospital, to deal with relatives/dependents of patients. A chart should be displayed at this reception counter indicating ward numbers where the patients are admitted, along with their names, coach number wise. At the entry to every such ward, a second list should display the name of the patient, coach number and the bed number inside the ward. Commercial staff and Welfare Inspectors on duty at that hospital should carry a list indicating the name, address and telephone numbers of relatives/dependents as given by the patient, and whether they have been informed or not. Arrangements should be made to inform the next of kin or a relative or friend of the deceased, in case identity of the person involved in accident becomes known. As each relative arrives, his name should be marked in the list against the passenger’s name. Reception counter should be provided with BSNL telephone with STD facility. There should be two mobile telephones readily available to be taken to patients inside the wards for making outgoing calls. Complete medical care of all passengers, including payment of medical bills till their final discharge, should be provided. 13.4 COMMUNICATION Telephone with STD facility should be made available to passengers to communicate with their relatives/dependents. BSNL/Railway telephones available at adjoining stations/cabins/towns shall be extended to the accident site. PCO telephones and other BSNL phones in nearby localities/villages/towns shall also be extended to the accident site by persuading owners of such phones. Payments for such telephone connections will be made from station earnings. Sr. DSTE should hire some mobile phones to meet the needs of stranded passengers. Wherever cellular phone connectivity is available, stranded passengers should be permitted to use these phones free of charge Passenger help lines are provided for enquiry service No. 1072 in all Divisions of SEC Railway in commercial control rooms for collecting information in Disaster or any other major train accidents. FACT phones should be available in all the ARTs. Laptop with Data card, FAX facility, INMARSAT must be available in each ART. BEGIN THE DAY WITH SAFETY AND END THE DAY WITH SAFETY 38 13.5 ARRIVAL OF RELATIVES/DEPENDENTS After a few hours, next of kin of the deceased and relatives/dependents of injured passengers start arriving at the accident site. Adequate number of display boards should be available on ARMEs/ARTs for being put up at the accident site. These display boards should indicate the direction of the Assistance Centre at site. These indication boards should be displayed near those areas where incoming relatives/dependents arrive and congregate. Periodic announcements on loudspeakers should also be made for guiding them to the Assistance Centre at site. 13.6 TAKING CARE OF RELATIVES/DEPENDENTS At the Assistance Centre at site, Commercial Supervisors & Welfare Inspectors should be available to guide the relatives/dependents. They should go through the reservation charts and list the dead/injured. Commercial supervisor or WI shall depute a railway servant to accompany the relatives/dependents to the hospital. A hired vehicle should be provided for carrying them to various hospitals and mortuary. The commercial supervisor or WI should stay with the relative until they have either found the injured passenger or identified the dead body. Thereafter, they should help them in completing all formalities at the Assistance Centre at site. CI and WI must be nominated for each ARMV/ART. 13.7 SINGLE WINDOW CLEARANCE Assistance Centre at site should provide single window clearance for all legal formalities and paperwork. It should provide the following facilities: Reservation chart for locating the name. List of dead and injured along with the name of hospital. A vehicle, to take the relatives to various hospitals or mortuary. Railway doctor for issue of medical Death Certificate. Govt. doctor for issue of post mortem clearance. Municipality official for issue of Death Certificate. Local police for handing over of dead body. Claim counter for payment of ex-gratia and issue of Claim Compensation Form. Counter to help in performing last rites in case relatives/dependents decide to cremate the body there itself. Pass counter for issue of return journey pass. Return journey facilitation counter to make arrangements for return journey. 13.8 WITHDRAWAL FROM STATION EARNING Money can be withdrawn from station earning with personal sanction of a senior scale officer. Station pay order (withdrawn from station earnings) should be duly signed with official designation, indicating his name. Proof that the payment has been made and the reason for payment should be kept payee with revenue stamp. The accountal should be submitted to account officer within 15 days from the date of withdrawal. (Para 2425of Indian Railway Commercial Manual) SAFETY RUNS TRAINS ON RIGHT PATH.. 39 13.9 DEPARTURE OF RELATIVES/DEPENDENTS OF DEAD AND INJURED Assistance Centre at site should have counters to help the relatives/dependents plan their return journey. Personnel branch staff at the Assistance Centre at site should be available for issuing complimentary passes for their return journey. Reservation of berths should be provided on trains. Extra coaches should be attached to trains going to the destination station for the next two or three days. These extra coaches should be brought in locked condition form the originating station. To convey any other information which is of use to passengers. To convey specific information which is of use to relatives/dependents of dead and injured passengers. To create a positive public opinion. To create a healthy relationship with the press and electronic media. DUTIES OF PUBLIC RELATIONS ORGANIZATION 14.3 CPRO & Sr.DCM and his team will collect whatever information is available and release it to the media within 60 minutes of intimation of accident. The information shall include telephone numbers of Helpline Enquiry Booths. Photographers with digital and video cameras should also be taken to the accident site. Responsible PR supervisors should be deputed during night shift for interacting with the media, if necessary. CPRO & Sr.DCM will organize press briefings at fixed timings. PR organization shall monitor various important media channels to keep track of media reporting. Suitable corrections/clarifications may also be issued, if required. SPOKES PERSON Only GM, DRM CPRO and & Sr.DCM are competent to interact with press and electronic media. Apart from the above, any other officer authorized by GM & DRM is competent to interact or give interview to press and electronic media. They should ensure that only factually correct and confirmed information is relayed. No inflated or exaggerated version of any fact should be relayed to the media. No Railway man shall express or voice any criticism, or express his personal opinion or views about the accident, at any point of time. 14.4 INFORMATION TO BE RELAYED TO PRESS AND ELECTRONIC MEDIA Information to be given to media can be broadly segregated in to following categories: 14.4.1 14.4.2 ACCIDENT Nature of the accident, i.e. date, time, exact location, train number, number of coaches involved, etc. Details of how the accident most probably occurred. Prima-facie cause of the accident will be relayed to media only with the approval of GM. Sabotage, even if suspected will not be relayed to media, without approval of Railway Board. Regular reports regarding progress of Rescue & Relief work. Expected date and time of restoration. UNINJURED PASSENGERS Steps taken to provide beverages, refreshments and first aid treatment to uninjured passengers. Steps taken by railway for clearance of uninjured passengers. SERVE SAFETY AND SAFETY SERVES YOU. 41 14.4.3 14.5 Expected time of departure of front portion of the affected train. Expected time of arrival at the destination. Expected time of departure of rear portion of accident involved train Its diverted route and expected arrival at the destination. In case empty coaching rakes have been arranged, then details of the same. DEAD AND INJURED PASSENGERS: Steps taken by Railway to render immediate medical attention. Number of passengers rescued. Breakup of the injured passengers, both grievous and simple. Name of the hospitals where injured are being treated. Approximately how many patients have been admitted in each of these hospitals. Names of injured passengers. Communication facilities like cell phones, STD phones provided at these hospitals. Payment of ex-gratia. Facilities offered to relatives/dependents of victims, including free pass for journeys. Special trains being run for bringing relatives/dependents of dead and injured. Number of dead bodies recovered and number of bodies identified. Identification of dead bodies takes much longer since either: They were traveling alone or; Their companions are injured and are not in a position to identify them, or; Their companions have also injured/dead. Under such circumstances it is possible to identify dead bodies only when relatives/dependents arrive. This aspect of identification of dead bodies and reasons for delay should be explained to the media. AUTHORITY TO DEAL WITH THE MEDIA Refer DRM/BSP L.No.PR/BSP/30/2012 dated 29.09.12 in accordance with “MANUAL FOR PUBLIC RELATIONS DEPARTMENT (FIRST EDITION-2007) CHAPTER-IV” 1.1 1.2 1.3 1.4 At the Railway Board Level, only ministers, Chairman, Members, Secretary Railway Board, Director Public Relations (DPR), Director Information & Publicity (DIP) or any other Officer(s) especially authorized by the Minister of Railways may give information or be accessible to the representatives of the media. Any other officer, if approached by the representatives of the media is to refer them to the DPR (the official Spokesperson for the Ministry of Railways) or in his absence to the Information Officer. The Chairman is to be kept informed of the press conferences, if any, being held by any member of the Board. At the Railway/PU Headquarters Level, the General Managers and the CPROs are authorized to meet the media in a formal Press Conference or informally depending upon the importance of the nature of the information to be given. However, the General Managers may especially authorize the PHODs to give information or be accessible to the representatives of the media. Any other officer, if approached, by the media, is to refer them to the CPRO. At the Divisional Level, Divisional Railway Managers (DRMs) are permitted to meet the representatives of the media approaching them for factual information on specific subjects. They may also send news items concerning the working of the Railways (particularly in their Division and other matters of loco interest) to the media directly or through the local office of Press Information Bureau( if available). DRMs may also hold press conferences occasionally. However, in respect of queries on wider policy matters concerning the Zonal Railway or the GM/CPRO or DPR/Railway Board. In order to ensure that no unauthorized information/material is supplied to the press, the DRMs should generally not delegate this work to a Divisional Officer except to ADRMs. All such information/material should have DRMs approval before it is supplied to the press. Services of PROs ( wherever positioned) may be utilized for the purpose. It is made clear that no unauthorized person should speak to or interact with the media as it may amount to unbecoming of a railway servant. In this connection, the provisions relating to official documents and responsibility of railway servants (contained in Rule-11 of Railway Servants (Conduct) Rules-1966) may also be kept in view. ALWAYS THINK AND ENSURE SAFETY. 42 2. DISSEMINATING INFORMATION TO MEDIA, MEDIA-FEEDBACK & REJOINDERS 2.1 Notices of all important meetings, conferences, visits of delegations and VIPs shall, as a matter of routine, be sent to the DPR/CPROs for organizing appropriate media publicity. Even if the executive department feels that the subject matter does not warrant publicity, the decision to publicise in the media or otherwise shall be taken in active consultation with the CPRO. 2.2 All PHODs/DRMs must feed the PR Department regularly with information/material about activities, projects and achievements for making suitable ‘handouts’ and ‘features’ or for arranging audio-visual coverage as and when such information is available with them. 2.3 DPR/CPROs will be responsible to develop an appropriate data bank on various subjects for their use from time to time on the basis of information received and get it updated by executive departments from time to time. 2.4 An annual action plan of PR events may be made at the beginning of each financial year on the basis of major works being undertaken on the Railways and the performance targets laid down for yarious executive departments. In addition, the PR Department should project specific items of achievement which would be undertaken during the year and keep the concerned departments advised in advance for inflow of data at appropriate time. 2.5 As a routine, Pr Department should ensure that concerned departments are fed with extracts of press reports deserving attention of the concerned PHODs/DRMs. Rejoinders, if necessary on any new report, must be issued within a week of its publication. If need be, some of the information required for such rejoinders can be collected even on telephone from the concerned executive Departments to avoid delays. PR Department as well as the concerned executive Departments shall be jointly responsible for timely rootials of undue press criticism. 3. PUBLICITY DURING ACCIDENTS/OTHERS UNUSUAL OCCURENCES 3.1 In the event of accidents, resulting in damage/causalities, the image of Railways invariably suffers because of adverse reaction in public and media. In such situations, Railways must display greater responsibility not only in relief and rescue operations but also in interacting with the media with correct and updated information. 3.2 Sometimes, newspapers publish contrary versions relating to any accidents attributing them to railway sources. It is, therefore, necessary that only the authorized officers (mentioned in Para 1.1 to 1.3) interact with the media. Unauthorized person should not speak to the media (as mentioned in Para- 1.4) >>>>>>>>>><<<<<<<<<< SAFETY HAS NO HOLIDAY. 43 Chapter:-15 DISASTER INFORMATION FLOWS AND ALERTS OF DISASTER 15.1 CATEGORIZATION OF ALERTS Specific hazards have different categories of Alerts. Accordingly, a uniform system has been devised by categorizing each type of alert in stages – Yellow, Orange and Red. 15.2 ACTION PLAN FOR COMMUNICATION OF ALERT MESSAGES Whenever a crisis is about to be faced, Government of India has laid down systems for warning its respective departments through an ‘Alert’. It should be understood that mere issue of an ‘Alert’ (Yellow or Orange) is not an indication of the occurrence of a disaster. This only signifies the existence of a crisis for which provisions of the Crisis Management Plan would come into operation. 15.3 THE ACTION PLAN FOR ALERT MESSAGE LAYS DOWN AS UNDER FOR RAILWAY. Category Minor Medium Major 15.4 Stage Yellow Orange Red ACTION ON DIVISION/ZONES ON ORANGE/RED/ ALERT 15.5 Description 50 or less casualties (inclusive of death and injuries). 51-99 deaths 100 or more deaths, or where additional assistance is sought by the Ministry Railways. On the issue of an Orange Alert (or of a higher level) the Responders have to be activated as required for relief etc. as under:Mobilization of Gang men Hospitals to mobilize Doctors and Para-medical staff Civil Defence units RPF and RPSF deployment Scouts and GUIDES Operating and manning of the disaster control room Coordination amongst various stake holders through advance warnings Communication system to be ensured and back ups to be in readiness for immediate use when required. In case existing railway staff may not be able to maintain train services to be operational, the TA units have to be mobilized. It takes 2-3 days for the deployment of the TA unit after issue of their mobilization order; hence an advance warning is of essence. MONITORING/REPORTING OF EFFECTS OF DISASTER – The safety dept. in the Board would be given information regarding Orange/Red Alerts. On the declaration of an incident as a Disaster by a State Government or District Administrator or even by the GM/AGM of the Zonal Railway, the CSO would give time to time updates to the Safety Control in Railway Board of the situation. Assistance of other departments would be made available by the GM to the Safety department on the Zonal Railways. Divisional set up will give feed back to HQ accordingly. THE TIME TO PREVENT ACCIDENTS IS BEFORE IT HAPPENS 44 15.6 IN CASES OF SPAD (Refer Procedure Order issued by DRM on date 11.09.12 ) Immediate Response whenever any SPAD Case is reported. Following action shall be initiated by all concerned, as soon as above subject matter is reported. Relaying of Information from the site 1. The ASM concerned, whosoever comes to know about any SPAD case in his sectional jurisdiction, shall immediately take the following action :a. He shall inform the Section Controller about the incident and communicate current status of the train & signal i.e. Blank Signal/ ON Signal, Train Stopped or not, etc. b. He shall make a diary entry of the incident clearly narrating the time and information about it, along with all names of concerned running and station staff present. Action Required to be taken in Control 1. Section Controller should immediately inform to Dy. Controller (M/Line or CIC). 2. Dy Controller (M/Line/CIC) shall perform following activities :a. b. c. 3. Inform the concerned controllers such as TLC/DPC, Test Room, TPC (if switching off of the OHE is required), BT Control, etc. In form the concerned B.Os (Sr.DOM, Sr.DSO, Sr.DEE (OP)/Sr.DME, Sr. DSTE, Sr.DEN, Sr.DEE/TRD (if required) and CMS/BSP. Depute DTI to proceed to the site. TLC/ DPC shall perform following action :a. b. Inform Sr.DEE(OP) or the Sr.DME as well as concerned sectional officers of department. He shall, if possible, arrange for changing of the Crew and Loco (in consultation with the Dy. Controllers) of the involved train, immediately at the same station or next block station as soon as SPAD case is reported to him. c. He shall depute 2 CLIs to proceed to the site, d. He shall, immediately depute the Teloc Inspector for decoding the speed of the locomotive. e. He will chase with medical department for arranging blood test of the crew involved. 4. Test Room shall perform following action :a. b. c. Inform to the Sr. DSTE and concerned sectional officers of S&T department. Take out a print-out, if possible, of the Data Logger report, Obtain following information regarding signal :i. ii. iii. iv. v. Signal blank or not, Signal passed at Danger or not, Red lamp protection, if feasible, was intact or not. Working condition of the panel of the concerned stations. Call details of both LP and ALP to be collected. A SHORT CUT IS OFTEN A WRONG CUT 45 5. Carriage Controller shall perform following action :a. b. c. 6. Arrange to obtain record of any other relevant records from site such as casualty, damages, etc. Action By Site Supervisors 1. The site supervisors viz., DTI(s)/ CLI(s)/ SSE(C&W)/ BSI/ Safety Counsellor (s) and/& other supervisors shall arrange and ensure following action at site :a. b. c. Statements of all the involved staff such as ASM, Guards, Crew, etc shall be taken on spot immediately. Joint testing of brake power of the train and Locomotive. Conduct Breath Analyzer test. The BA test must be done by at least two breath analyzers, simultaneously with three continuous blows of breath on each instrument d. Freezing of the Speedometer of the involved Loco and taking out the memory card of SPM. e. Must ensure collection of blood sample by medical staff at the location, where crew is relieved. f. Photographic evidence of SPAD if possible should be taken and submitted to Safety Branch. g. A neat sketch of the site shall be prepared clearly indicating the amount of overshooting and the location of signal and station. h. Ensure submission of gradient chart of the relevant section/station as well as details of caution orders imposed. i. Must ensure that signal defect register is seized and handed over to the Safety branch. j. Must arrange for a Signal visibility test to be conducted of the relevant station/ section in the desired direction and line, by signal sighting committee. k. Must ensure checking and verification of presence of signal staff or their working/maintenance staff at the relay room of the relevant station/ IBS. l. Fault logged in DDS of locomotive (Any microprocessor based loco) should be down loaded at the earliest. >>>>>>>>>><<<<<<<<<< MAKE SAFE WORKING A REGULAR HABIT 46 Chapter:-16 HOSPITAL DISASTER MANAGEMENT PLAN 16.1 AIM OF HOSPITAL DISASTER MANAGEMENT PLAN The aim of a hospital disaster plan is to provide prompt and effective medical care to the maximum possible, in order to minimize morbidity and mortality resulting from any MCE (Mass Casualty Emergency). 16.2 HOSPITAL DISASTER MANAGEMENT PLAN: There shall be on each Zonal Railways a hospital disaster management plan which will be based on the Indian Railways Hospital DM Plan issued by Railway Board. The Hospital DM Plan shall incorporate relevant items given in the DM Plan of the Railways. It should be clarified that: “The hospital DM Plan comes into effect only if the competent authority so authorized declares on the Zonal Railways an incident as a disaster. It can also come into effect if any central/State Government agency declares a major incident a Disaster and where the medical facility of the Railways shall be required to assistance”. Each Hospital has to evolve it’s own plan and it has to be revised from time to time as each experience will bring new perspectives. 16.3 OBJECTIVE AND GOALS OF A HOSPITAL DISASTER PLAN – The hospital disaster plans should address not only mass casualties that has occurred away from the hospital, but should also address a situation where the hospital itself has been affected by a disaster- fire, explosion, flooding or earthquake. 16.4 16.5 Disaster Committee. Other vehicular accident and aircraft emergencies. Bullet and blast injuries. Collapse of building. Food poisoning – gastro enteritis. Unprecedented disaster epidemic – dengue fever, swine flu, viral hepatitis etc. Natural calamities – flood, earthquake, cyclone, land slides. Unprecedented chemical or biological disaster. Unprecedented terrorist strikes. Any other like drowning etc. 47 16.6 Headquarter for Disaster Plan Co-ordination Control room: Sr. DMO (Admn.) Tel: no. 62478 (Rly.) & 247078 (P&T) round the clock. 16.7 Information and Communication One hot line from police head quarter in the Zonal/Divisional/Hospital control room. Direct arrival of casualty without any prior intimation. Details to be ascertained on the hotlines are Time and place of occurrence. Nature of accident. Approximate number of casualties. Source of information. Authenticity. 16.8 Activating the Plan At receipt of information from authentic source, the duty officer will activate the plan and inform the MD, CMS, Sr.DMO (Admn.), Sr. DMO (Genl.), ANO and OC/RPF. 16.9 Reception centre In reception area one doctor, one paramedical staff and one volunteer from SJAB will be there to record address, telephone No. and destination in the hospital after examination of the patient by the doctor, the patient will be sent to first aid centre or priority area. For minor load: The present casualty will function as the reception area. For moderate load: Entrance hall of indoor hospital will be converted into reception area. For major/Heavy load: Nurses duty room and verandah of ward I will be utilized for reception in addition to above. Pollice, RPF personnel will act as traffic controllers directing the patient and relatives to the respective reception centres. 16.10 First Aid and Sorting: Triage For minor load: Existing casualty medical team will function for first aid and sorting. For moderate load: The centre will be manned by two teams each consisting of: One General surgeon. One Orthopaedic Surgeon. One Physician. One Anaesthetist. Two Sisters. Two Hospital Attendant. One Safaiwala. Two stretcher bearers. For major/heavy load: Centre will be manned by four teams each consisting of: One General surgeon. One Orthopaedic Surgeon. One Physician. One Anaesthetist. Two Sisters. Two Hospital Attendant. One Safaiwala. Two stretcher bearers. If adequate number of specialists (General Surgeon/ Orthopaedic surgeon/ Physician/ Anesthetist) is not available then duties of General Duty Medical Officers may be obtained for forming the clinical team. Help of private/government specialists may be obtained. Available specialist may help different teams. If required specialist from adjacent zone may be mobilized. 48 16.11 The Responsibilities of the First Aid Centre will be: Quick sorting of casualties into: Priority one: Needing immediate resuscitation. Priority Two: Immediate surgery. Priority three: Needing first aid and possible surgery. Priority four: Needing only first aid. Action: Priority one: Will be attended to in casualty and if need arises will be sent to ITU/HDW. Priority two: Will be transferred immediately to major O.T. Priority three: Will be given first aid and admitted if bed is available or transferred to other hospital. Priority four: Will be given first aid and discharged to home. The area marked for holding ward: Corridors of ward V & VI. Brought in dead or those who may die while receiving/resuscitation will be segregated. Temporary morgue for keeping dead bodies will be created in the verandah opposite the ward III. Necessary identification and handing over of bodies to the police will be done in this area. No bodies will be handed over to the relatives without medico legal clearance. This will function under the care of Sr.DMO (Genl.). 16.12 16.13 16.14 Additional Bed Space. In addition to area marked on ward V & VI extra bed space will be created as follows: Utilization of all cabins with extra bed facility. Any vacant bed will be requisitioned by the MD/Sr.DMO (Genl.) for this prupose. By discharging following category of patients. Convalescing patients needing only nursing care. Elective surgical cases. Patients who can have domiciliary care or OPD advice. Conference hall on first floor may have to be used temporarily. Linen Stores : A side room in ward IV is earmarked for this purpose. Following stores should be transferred to that room from stores: Mattress – 30. Bed sheets – 100. Blankets – 80. Pillows and cover – 80. Patient clothing – 50. IV stands – 20. Oxygen cylinders – 15. If additional bed sheets, blankets, pillows and cover are required ANO may demand from Commercial/Store Department. Drugs and equipment: Pharmacist main store will be called at once to open the store. As an immediate measure the buffer stock earmarked in casualty will be utilized. All essential drugs will be stocked by the medical stores and issued on orders of MD, Sr.DMO (Stores), duty casualty medical officer. Dressing material and items of surgical stores are similarly kept in reserve. A dozen emergency trays containing life saving drugs will be kept ready in medical stores. For first few hours and for immediate use the drugs will be requisitioned from emergency stock lying with the sister in-charge of casualty. Approximately 400 bottles of IV fluids/crystalloids are kept available by the store in-charge. 49 16.15 16.16 16.17 16.18 16.19 16.20 16.21 16.22 16.23 16.24 16.25 Emergency Blood Requirement: Volunteers and voluntary organization will be approached to donate as much blood as possible. Liason with Blood Bank officer of Chattisgarh Institute of Medical Sciences (CIMS) and Apollo Hospital, Bilaspur to obtain required blood units. Staff : Medical Staff: All paramedical staff of OPD in addition to members of regular clinical units will be asked to render help to assist the clinical staff in managing the casualties. The duty roster of regular consultants and standby doctors is to be made available in control room. Nursing Staff: A pool of nursing staff will be created by Assist. Nursing Officer so that nursing staff is available at short notice. Other Staff : Health Inspectors and Welfare Inspectors will be deployed to help medical team for documentation and identification at registration counter and patient’s relatives. Volunteers: Volunteers will be invited by the coordinated efforts of MD and SJB officer. Help of St. John Ambulance Brigade, Civil Defense and Scouts and Guides may be obtained. Documentation Centres: For small/minor load of casualty: Documentation shall be done at casualty itself. For moderate load of casualty: It is to be established in nurses room of ward I. For major/heavy casualty : Verandah of ward I shall be utilized for this prupose. The staff working at registration counter and nursing staff will be utilized for documentation and identification. Volunteers may also be used for this purpose. Hospital Security: Security of staff, patients, hospital building and equipment being of paramount importance during such disasters, the OC/RPF is to be requested to tune up and organize the security arrangements for this purpose. For minor casualties hospital security personnel may be adequate but for all moderate to major loads help of police and RPF must be obtained. Food Service: Supply of nourishments to the patient and emergency duty staff will start immediately by the staff of the dietary services under the direct supervision of dietician and clerk in – charge of kitchen. Most of the patient for 24 to 48 hours will be using only liquid or semi-solids. By then efforts can be made for supply of proper meals. Information Services: Faculty of hospital administration will function as information officer and all information to press, radio and other media, individuals, organization, government or otherwise will be issued by him. He will get prior clearance from competent authorities before issue of such information. Engineering and Maintenance Service: The engineers will make sure that water and electricity is made available without interruption. All standby electric power generators will be regularly checked, inspected and maintained in excellent serviceable condition. Alternative water supply source will be made available. Health Inspectors will ensure round the clock maintenance of general cleanliness and sanitation in and around the hospital. Comforts to Patients Relatives: In case of major disaster the hospital authority may ask for certain buildings like Officers Club, Sanskritik Niketan for providing the comforts to the patient relatives. Discharge Procedure: After appropriate treatment the casualties fit to be discharged shall be discharged to go home or to other hospital for convalescence. For all cases discharged the destination will be noted by the hospital and police informed. Success of Plan: Disaster is an emergency situation. Timely help to every individual is needed to make this plan a success to reduce the mortality and morbidity. In such state of affairs the individual and personnel consideration take low priority in the face of duty to the profession for sake of amelioration of human suffering. 50 16.26 Conclusion: Disaster management involves a host of multi discipline agencies of which medical relief is one of the most important one. There can be no tailor made disaster plan for the hospital and it has to be revised from time to time as each experience will bring new perspectives. Finally, it must be understood that a disaster can occur anywhere and at any time. It is no respecter of circumstances. It strikes with suddenness and fury and has curious tendencies of choosing the most in-appropriate moment. To deal with such sudden influx of a large number of casualties, quantitative expansion of hospital has to be pre-planned. >>>>>>>>>><<<<<<<<<< THE TIME TO PREVENT ACCIDENTS IS BEFORE IT HAPPENS 51 Chapter:-17 ROLE OF SECURITY DEPARTMENT IN DISASTER MANAGEMENT 17.1 SECURITY ON INDIAN RAILWAYS The security of Indian Railways is being managed by 3 agencies- the Railway Protection Force, State Police and the Government Railway Police. State Police and Government Railway Police function under the administrative control of respective State Governments and their role is prevention and detection of crime except those covered by Railway act and the Railway property unlawful possession act and tackling law and order problems. Due to this coordination amongst the different agencies is very essential to handle Disaster situation effectively. Railway Protection Force is an Armed Force of the union constituted by an act of parliament (the railway protection force Act 1947), for the protection and security of railway property, passenger area & passengers and for matters connected therewith. 17.2 ROLE OF RPF IN DISASTERS:In case of any disaster especially serious train accidents, fire incidents, explosion in trains or on railway premises, terrorist acts, hijacking of train etc. RPF has to play lead role on coordination with other department of Indian Railways and various agencies of State and Central Government. In case of CBRN Disasters, or a natural calamity, RPF has to provide support services in rescue, rehabilitation and mitigation efforts. RPF has a major role in crowd control and arranging fire fighting infrastructure by coordinating with the State Governments/District Administration. The deployment of the RPF may be done on need basis to provide relief, rescue and rehabilitation consequent to a Terrorism Disaster. Dog Squad may need to be deployed even for preventive checks against terrorist activities. 17.3 INTEGRATED SECURITY SCHEME:An integrated security scheme has been sanctioned for installation at 195 stations of Indian Railways. The system envisages multi-layered surveillance of vehicles, luggage and passengers in station premises. The system comprises of following broad categories: 17.4 Internet protocol based CCTV surveillance system with intelligence video analytics, Access control, Personal and baggage screening system, Bomb Detection and Disposal system. CROWD CONTROL AND MANAGEMENT:For better effectiveness, RPF, GRP and District Police have to act in a synchronized manner in consultation with magisterial authorities. One of the intelligence video analytics to be incorporated in the integrated Security System is related to signal for crowd density within station premises when it exceeds the prescribed limit. This will enable RPF personnel and railway authorities to get timely information when heavy crowd builds up within station premises and plan follow-up action. Pictures stored on CCTV system will be of immense help in identifying miscreants and in ensuring effective legal action. ALWAYS BE ALERT AND AVOID ACCIDENT 52 17.5 EXPLOSION IN TRAINS AND RAILWAY PREMISES:One of the key components of the integrated security system is explosive detection and disposal. It provides for effective detection and disposal capability with RPF. Explosive detection and disposal, being a highly skilled and challenging job, requires Bomb Detection and Disposal Squad comprising of personnel. Such RPF personnel have been identified on each Zonal Railway and they are being trained in phased manner by National Security Guard (NSG). Preventives measures to be taken in such situation, have been separately circulated vide Security Directorate Secret letter No. 2003/Sec (Spl.) 200/14 dated 16.01.2008. 17.6 TERRORIST ACTS & HIJACKING OF TRAINS:Procedures have been outlined in the Crisis Management Plans of the Government of Indian, of the Ministry of Home Affairs and of the Ministry of Railway to tackle such situations. Above mentioned secret documents are available with concerned Authorities and action has to be ensured in accordance with the provisions mentioned in the above mentioned plans. Ministry of Home Affairs is the Central Nodal Ministry to tackle hostage to terrorist situations requiring specialized handing. National Security Guard (NSG) has to be requisitioned in such situations. Crisis Management Plan of the Ministry of Railway envisages management of such crisis by the National Crisis Management Committee (NCMC) and Crisis Management Group (CMG) at the Railway Board level and by the zonal management at the zonal level. Coordinated efforts have to be ensured by all security agencies present at the spot. Senior most officials available at the spot shall handle situations in accordance with conditions of the crisis at local level and instruction received from concerned Crisis Management Group at Zonal and National levels. Quick Reaction Teams (QRTs) of RPF personnel should be available round the clock at major stations which will be immense help to tackle such situations during initial phases especially in cases of terrorist attacks. >>>>>>>>>><<<<<<<<<< A DANGER FORESEEN IS AN ACCIDENT PREVENTED 53 GOVERNMENT OF INDIA MINISTRY OF RAILWAYS RAILWAY BOARD No. 97/Sec.(Spl)/200/38 New Delhi, Dated – 31.07.1997. STANDING ORDER No. 34 All the Chief security Commissioners/RPF. Sub: Role of RPF in the event of Railway Accidents and other calamities. 1. While visiting one of the Railway accident sites, it was felt that RPF were not well organized to handle emergency situations. They are, to some extent. Confused about the role they have to play in the event of Railway accidents or incidents causing huge loss and damage to Railway property and injury to passengers in cases of sabotage or natural calamities. Since these are extraordinary situations, utmost confusion and chaos prevails at the scenes of accidents/incidents adding to the agony of injured persons and serious delays in rescue and restoration operations. RPF would normally be the first uniformed force to arrive at the scene of accident, and therefore, they have to play a very important role. 2. First Response: First information about any calamity involving Railway trains or Railway premises, will normally be received by the nearest RPF Post/Outpost. The person receiving such information should muster the maximum available manpower within the shortest possible time and dispatch them to the scene of accident by the quickest means. After dispatching the immediately available force, the Post/Outpost in charge should requisition additional manpower. He should also simultaneously pass on the information to the senior supervisory officers and the Control Rooms. 3. Reinforcement: effort will be made to get the reinforcement from the neighboring posts/outposts, Reserve Line, divisional Headquarters or Zonal Reserve in case any RPSF battalion or company is located in the vicinity, men can be requisitioned from there for dealing with such emergent situations till additional force is available from other sources. 4. Equipment: While sending reinforcement, it should be ensured that the necessary equipment required for rescue, recovery and protection of the scene of incident are provided. Such equipment should include. (i) (ii) (iii) (iv) (v) (vi) (vii) Torches and other lighting arrangements, if it is night time. Nylon ropes and poles for segregating the affected from unwanted visitors and spectators. Loud-hailer for making announcements. Stretchers and first aid equipment. Wireless sets for inter-communication. Cameras for photographing the scene (both on negative and slide films) Video recording of rescue and salvage operations and connected administrative arrangements. Note: S. Nos. (vi) and (vii) will be useful for departmental presentations and for training of RPF personnel. 54 5. (a) (b) (c) Action at the scene of incident: The senior most RPF officer available at the scene of incident will assume control and immediately start the following action: Segregate the area of incident by establishing temporary barriers by use of nylon ropes or any other makeshift device available at the scene. It should be ensured that the on-lookers and spectators do not enter the affected area to disturb the scene or hamper the rescue operations. Baggage of passengers should be isolated and protected and consigned goods should be taken care of till they are handed over to claimants or taken over by railway authorities. RPF personnel should respond to any call for assistance to rescue victims and transport them to the nearest hospital. A record of casualties sent to hospitals should be maintained. Note: RPF officers will maintain close liaison and harmony with the officers of various departments of the railways, the GRP. Local police and officers of Civil Administration while attending to (a), (b) and ( c ) above. (d) Hourly sitreps will be sent by the officer at the scene of incident to the Divisional/Zonal Control room giving the latest situation. (e) A temporary RPF ASSISTANCE POST (shed or tent) with proper Board should be established at a conspicuous location so that people needing help could approach the RPF. If the operation continues for a longer period, effort should be made to install a temporary telephone connection through the Railway Telecom Department, so that the information is passed on quickly. A log book should be opened and minute-to-minute progress of action by RPF on the lines indicated above, recorded. (f) 6. The senior most officer available at the scene of incident will also ensure proper documentation about the number of persons injured or dead, giving their identity and addresses, if available. In case any queries are made by the friends or relatives of the injured/deceased they should be properly guided. After the rescue, restoration operation is complete a cassettes and photographs of the scene of incident will be retained by the CSC in his office and will be properly catalogued and preserved for future reference. The Hindi version of the standing order will follow. The SO should be got translated into the regional languages and supplied to the Posts/Outposts for display on the Notice Boards. The RPF personnel should also be briefed on the contents of this SO during Roll Call/Parades >>>>>>>>>><<<<<<<<<< SAFETY FIRST AND ALWAYS 55 Chapter:-18 CRISIS MANAGEMENT PLAN 18.1 INTRODUCTION Cabinet secretariat have issued this confidential document earmarking nodal ministries/departments for the respective Crisis situations with instructions that they will further prepare their CMPs to deal with all crisis situations occurring on their systems. 18.2 Definition:Crisis:- A ‘’Crisis’’ is an unstable time for an organization, with a distinct possibility for an undesirable outcome. This undesirable outcome could interfere with normal operations of the organization, it could damage the bottom line or it could jeopardize the positive public image. In railways crisis is discussed which arises out of fires, floods, tornadoes, earthquakes, bombing etc. 18.3 Crisis Management:- Crisis Management is the process by which an organization deals with a major event that threatens to harm the organization, its stakeholders, or the general public. Three elements are common to most definitions of crisis a) a threat to the organization, (b) the element of surprise, and (c) a short decision time. 18.4 In contrast to risk management, which involves assessing potential threats and finding the best ways to avoid those threats, crisis management involves dealing with threats after they have occurred. It is discipline within the broader context of management consisting of skills and techniques required to identify, assess, understand, and cope with a serious situation, especially from the moment it first occurs to the point that recovery procedures start. Crisis Management consists of: 18.5 Methods used to respond to both the reality and perception of crises. Establishing metrics to define what scenarios consititute a crisis and should consequently trigger the necessary response mechanisms. Communication that occurs within the response phase of emergency management scenarios. Crisis Management methods of an organisation are called Crisis Management plan. Types of Crisis:There can be broadly 4 types of crisis situation which the Ministry of Railway may be confronted with: a) National level crisis developed in the Railway and is specific to a railway which is to ne managed with the help and assistance of other Ministers. All Indian Railway Strike is only such crisis for which Ministry of Railway is the Nodal Ministry. b) National level crisis affects the country including Railway and different Ministries/Departments have to help and assist each other based on their strengths. Cyclone earthquake etc. can be such crisis where railways have to assist by running special trains. Ministry of Home Affairs is the Nodal Ministry but Railways have to maintain liaison and flow of information for assistance to restore the affected railway system. Further for Terrorism/Security related Crisis, Ministry of Home affairs has to assist railway. c) Crisis situation which is not a national level crisis affects railway system which is to be managed with help and assistance of other Ministries/departments. Chemical explosion in train, fire in train, train falling in river, etc may be such situations. d) Crisis situation which is not a national level crisis affects railway system which can be managed with the help op internal resources from the railway only. in such crisis where railways have to help other ministries by way of rail transport. Ministries concerned will make their own Crisis Management Plan bringing out the assistance that the Railway will be required to provide to them. 56 Chapter:-19 SCHEDULE OF POWERS 19.1 SCHEDULE OF POWERS OF OFFICERS TO INCUR EXPENDITURE TOWARDS RELIEF MEASURE IN CASE OF SERIOUS ACCIDENT: (MISC. The subject matter has been reviewed by the Board and the following revised powers are delegated to the Zonal Railways. (i) (a) (b) (c) GMs/DRMs have been delegated the powers to rescue helicopter/aero plane to reach the site of serious accident for rescue operation expeditiously. In addition, powers are also delegated to requisition air support to dispatch the rescue trams to the site of the accident It is difficult to stipulate exactly the circumstances under which they may exercise these powers, it has been decided to leave this to the discretion of GMs/DRMs. However, broadly these may cover the following type of cases: Where more than 10 casualties (deaths-cum-serious injuries) are feared and it is difficult for these officer to reach the site within reasonable time. Where heavy damage is caused to railway installations in sensitive and tension filled areas ( e.g. wreckage of track bridges, etc, through bomb blast, other means of sabotage, etc. Where public reaction in case of late arrival of senior officers at site is likely to be highly adverse. Normally, in case of an accident of an accident only one helicopter should be requisitioned by a Zonal Railway, except when there is a serious passenger train accident involving several casualties when it is essential for both the General Manager and the Divisional Railway Manager to reach the site at once to satisfy the public and the Press. However, for dispatching the rescue teams to the site of the accident, separate helicopter/aero plane may be 57 (ii) 2 requisitioned, if so needed. The GMs/DRMs may exercise the above powers personally and may not be delegate these powers. Zonal Railways are further empowered to requisition helicopter/aero plane to evacuate injured and dead in the event of serious accident. GMs may personally exercise these powers and may not delegate these further. The above, instructions on the subject supersede all the previous instructions issued vide referred letters. This issues with the concurrence of the Finance Directorate of the Ministry of Railways. Extracts of Air Force instruction No. 9/83 regarding Regulations Governing Flights and Airlifts. (Auth. No.2002/Safety-I/6 dated 05.02.06) Extracts of Air Force instruction No. 9/83 regarding Regulations Governing Flights and Airlifts in IAF Aircraft within Indian Territory is reproduced as under: Para 9 (a): The use of IAF aircraft for civil agencies may be authorized by the chief of Air Staff on payment at rates which will be fixed by the ministry of Finance (Defence). Such flights may be provided on written demand for work like aerial photograph, survey and other purpose, survey of air routes, carriage of mail, drop of supplies, carriage of urgently required stores, leaflet dropping, relief and reconnaissance flights, carriage of Para military police forces etc. “Such flights as errand of mercy, on payment at chartered rate without detriment to Defence requirements, may also provided for evacuation of seriously sick or injured civilians from inaccessible areas at the written request of the state Governments, Union Territory Admn, and other Central Ministries ? Departments, as a life saving measures on an undertaking by such authorities that it is neither feasible by them nor any other means of civil air/surface transport is available for such evacuation and the evacuation by the IAF will be at their risk and cost “. The ministry of Defence may sanction, on payment, urgent movement of civilian Govt. servants and stores and equipment in IAF aircraft for various Ministries of the Central Government and for the State Government provided aircraft are available without prejudice to service requirements. Para 9(d): Station Commanders of and above the rank of Wing Commanders may authorize special flights of aircraft under their control to meet written request from Central Ministries/Departments/State Governments/Administration of Union Territories for assistance in any grave emergency or natural catastrophe. They will immediately advice concerned Command Headquarters. Such flights are to be regularized under authority of the AOs C-in C Command/Group in due course. Attention is also invited to Railway Board letter of even number dated 30/15-06-2004 vide which GMs and DRMs have been delegated powers to requisition helicopter/aeroplane even from private parties (copy enclosed) to reach the site of serious accident for rescue operation expeditiously and to dispatch the rescue team to the site of accidents. Zonal Railways may take note of the above standing instructions and establish co-ordination with the appropriate Air Force authorities. RIGHT TO EMERGENCY CARE (Date of Supreme court Judgment: 23.02.2007 Case No. : Appeal (Civil) 919 of 2007) The Supreme Court had ruled that all injured persons especially in the case of road or rail accidents, assaults, etc, when brought to a hospital/medical center, have to be offered first aid, stabilized and shifted to a higher center/government centre if required. It is only after this that the hospital can demand payment or complete police formalities. In case you a bystander and wish to help someone in an accident, please go ahead and do so. Your responsibility ends as soon as you leave the person at the hospital.
