*The first 10 pages of these reports are included in the non-refundable research fee with an additional
cost of $0.25 per page thereafter. The minimal $15 non-refundable fee must be submitted with your
request. Please note that Audio/Video/Photo may not be available. Additional fees must be paid
upon completion of request, prior to release. All records are subject to denial based on a case by
case review. Juvenile and sex assault records will not be released without proper authorization. If
required, redaction is billed at $35.00 per hour. Notification of additional charges will be furnished to
the email or phone number provided. Requests are processed in the order they are received. Upon
payment of the appropriate fee, expedited orders will be filled in 3 business days. The records will be sent

GLOSSARY OF RECORDS REQUEST TERMS
Term:
Computer-Aided
Dispatch (CAD)
Definition:
Typed synopsis of the 911 calls related to the incident. This is not
verbatim or a transcription of the call.
Premise History
Report
A summary of all 911 calls made regarding a specific address. This
includes calls to the non-emergency line. This can go back to 2002,
and can only be searched in 4 year increments. The premise history
cannot search individual apartments within a complex.
This will include all Officer Narratives, Witness Statements, Initial
Report, and Supplementary Reports for the case.
Supplemental
Report
911 Call
This is an audio recording of the call made to 911 and taken by the 911
call taker. Permission to release must be obtained from the caller
before a recording can be released. If a Defense Attorney is requesting
this call in relation to a criminal case they are handling, they may fill
out the Confidentiality Agreement to receive the call.
Dispatch
This is a recording of the calls between the Police Radio Dispatcher
Recording
and the officers responding to a 911 call. The Dispatch Recording
contains information concerning the 911 call.
Audio/Video/Photo This can include: interviews, photos uploaded, and surveillance
CD
footage (depending on content). Please call if you are not sure if the
item you are looking for exists. One CD per request.
Please note: HALO is only available for use in criminal matters
(Criminal Defense attorneys looking to use HALO in their case). If
HALO is desired for non-criminal purposes then it must be requested
via Subpoena. HALO video expires after 29 days; the requestor must
call to verify if a HALO camera exists at a location, and send in a
written request for preservation of the existing footage, pending a
subpoena.
DUI Room Video
This is footage of a person who has been arrested for DUI and held in
the DUI designated holding cells at the Van Cise-Simonet Detention
Facility. Please note: this footage is only retained for 25 to 28 days
after it has been captured.
Other
This can include: Officer Log Sheets, or various other documents. If
you feel your request fits this category you must call to verify the
documents you are searching for exist and are available. Please note:
