tŚĂƚŝƐƚŚĞ>W^WƌŽŐƌĂŵ: It is a language enriched exercise plus socializaon program (LEEPS) for
people with Alzheimer’s disease and related demenas.
,ŽǁŽĞƐŝƚtŽƌŬ͗The LEEPS volunteer is paired with a parcipant and performs simple language acvies and basic exercises with people affected with mild to moderate memory loss.
These acvies are provided in the parcipant’s home and/or community. Key Responsibilies of Volunteers:
• Sasfactorily completes all required LEEPS training.
• Sets up weekly exercise sessions with parcipant.
• Safely performs physical exercises with parcipant in their home.
• Conducts appropriate language smulaon exercises with parcipant when exercising & resng.
• Plans and accompanies parcipant on socializaon acvies in the community.
• Records and reports all hours including travel me on monthly log form. • Reports any problems or concerns to the Demena Care Specialist.

January 2015 Menu for Meals on Wheels for the City of Augusta, Village of Fairchild, and surrounding rural areas
AND Senior Dining at the Augusta Senior Center, 601 W. Main St., Augusta, WI at 12:00 noon Monday through
Friday. To be eligible for meals you must be age 60 or older or the spouse of someone age 60 and older. If you are 60
years and older meals are offered on a donation basis of $3.75 per meal. If you are under 60 years of age meal cost is $8.80
per meal. All donations help assure no senior goes hungry in Eau Claire County. Quest Cards accepted. For more
