OFFICIAL RULES FOR THE “GRAND OPENING RAFFLE” The Assistance League® of East Valley “Grand Opening Raffle” is being conducted by Assistance League of East Valley, a non-profit 501(c)(3) organization under the laws of the State of Arizona. Ticket purchaser must be 18 years of age or older to participate and must have a valid Social Security number. There is no limit to the number of tickets one person may purchase. First Prize: Second Prize: Third Prize: $1,000 cash Two nights stay at the Arizona Grand Resort Michael Kors Purse Raffle tickets are $5 each or three for $10. Odds of winning are 1:1,000. Tickets may be purchased by the following methods: In person at the Thrift Shop (1950 North Arizona Ave, Chandler Arizona 85225), or from any Assistance League of East Valley member who is a ticket seller. Cash, check or money order (made payable to Assistance League of East Valley). Should the bank not honor the purchaser’s check, the ticket will become void and withdrawn from the drawing. All sales and tickets stubs must be received with full payment no later than 1:00 pm on October 20th, 2016 or before a total of 1,000 tickets have been sold, whichever comes first. By IRS declaration, ticket purchases for valuable prizes are not eligible for charitable tax deductions. The winner will be determined by random drawing. Only official raffle tickets will be drawn. Drawing will be held at the Assistance League of East Valley’s new Thrift Shop located at 2326 North Alma School Road, Chandler, AZ 85225 at the Grand Opening Event to be held on October 21, 2016 at 4:00 pm. Winners need not be present to win. Winners will be notified by via phone and certified return receipt mail within 30 days of the drawing, and winners’ name will be posted on the Assistance League of East Valley’s website (www.assistanceleagueeastvalley.org). Prize must be claimed within 30 days of drawing. Any unclaimed prize after that date will constitute forfeiture and be transferred to Assistance League of East Valley as a donation. The winners will be required to complete affidavits of eligibility and a publicity release, which must be returned within 30 days of notification at which time the prize will be released or an alternate winner may be selected. The prizewinner is solely responsible for all local, state and federal taxes. The Internal Revenue Service requires that winners of any prizes valued at $600 or more be issued a Form W-2G, and requires that 25% of total winnings be withheld for federal taxes on prizes of $5,000 or more. The First Prize winner understands, acknowledges, and agrees that the First Prize winner is receiving a prize and will hereby release and discharge Assistance League of East Valley and any additional sponsors, their employees, agents, officers, directors and legal representatives and assigns from any and all claims, liabilities, damages, losses or expenses arising from or caused by the award of such prize, including, but not limited to the payment of any and all taxes that are due. Assistance League of East Valley, its Board of Directors, and its agents assume no liability for lost, late, misdirected, mutilated, incomplete, illegible, or entries without payment which will be deemed invalid and disqualified. The decisions and rules of Assistance League of East Valley are final on all matters concerning the raffle. Void where prohibited by law. LIMITATIONS OF LIABILITY: Purchaser agrees to release Assistance League of East Valley, its directors, officers, members, and agents from any and all liability for injuries, losses, or damages of any kind caused by participating in the raffle or winning any prize or resulting from acceptance, possession, use, or misuse of any prize and each winner agrees to indemnify and hold Assistance League of East Valley harmless from any and all losses, damages, rights, claims, and actions of any kind arising in connection with or as a result of participating in the raffle or the winner's acceptance or use of any prize. DISPUTES: A raffle participant's sole and exclusive remedy for a breach by Assistance League of East Valley is limited to the return of the purchase price paid for his or her raffle ticket(s).
