Patient Information: Male, Age 76
Diagnosis: Stroke / Subdural Hemorrhage / Intracerebral Hemorrhage
History: This gentleman was hospitalized after experiencing a hemorrhagic stroke. After a two
month hospitalization, he was admitted in a comatose state to Aristacare at Meadow Springs.
Rehabilitation was initiated to improve his level of function one month after skilled nursing center
placement, as he began to wake from his coma.
Pre-Therapy Status:
• Arm Strength: Poor (2/5) throughout both arms.

Patterned Electrical
Neuromuscular Stimulation
(PENS)
• Head Control/Posture: Limited voluntary movement of the head and neck; required
positioning in a tilt-in-space wheelchair due to trunk weakness and poor postural control.
• Bed mobility: Dependent (100% assistance).
• Transfers: Dependent (100% assistance); required mechanical lift.
• Standing: Unable.
Therapy Information:

• Modality: Omnistim FX2 Pro Patterned Electrical Neuromuscular Stimulation
®
(PENS), Omnicycle Elite Motorized Therapeutic Exercise System.
• Frequency: 3x per week.

Exercise System
• Protocol: Upper Extremity Biphasic PENS neck protocol, alternating with Upper
Extremity Triphasic PENS thoracic protocol for postural reeducation. Omnicycle
Neuro Mode for upper extremity cycling to increase strength, range of motion,
and aerobic capacity.

• Duration: Ten weeks.
• Other Therapy Services Provided: Postural retraining, therapeutic exercise for strengthening, and bed
mobility/transfer training. Outcome:
•
Arm Strength: Poor (2+/5) in the shoulders; Fair (3+/5) in the elbows.
•
Head Control/Posture: Improved; able to move head freely to interact with his surroundings. Still positioned
in a tilt-in-space chair for pressure relief.
•
Bed mobility: Moderate (50%) assistance.
•
Transfers: Moderate (50%) assistance.
•
Standing: Moderate (50%) assistance to stand in the parallel bars.
This gentleman has made significant progress over his course of care at the skilled nursing facility. After his stroke, his
spouse sought out placement at this facility due to the availability of the Coma Stim Therapy Program. This program has
improved his alertness and mobility to a degree that has enabled him to return home with his wife and home health
services. Both the gentleman and his wife are happy with his outcome and the wife describes the care he received from
nursing and rehab as “Excellent!”
