Thank you for choosing Source Power & Gas LLC as your retail eletric provider. Enclosed you
will find the New Occupant Statement form.
Please be advised that this form along with a Proof of Occupancy document is required to
complete your service request for electricity. Acceptable Proof of Occupancy document
statements include:
(1) Copy of Signed Lease
(2) Affidavit of Landlord
(3) Closing Documents
(4) Certificate of Occupancy
(5) Utility bill in customer name dated within the last two months from a different premise.
