The materials necessary to conduct the Knights of Columbus 5th Sunday Rosary Program
are available through the Supreme Council Office. Order your kit early to guarantee receipt
of all materials in advance of your council’s Rosary service.
Complete the label below and submit it to the Department of Fraternal Services.
Please type or print all required information since this will also be used as the mailing label for
your council’s kit (Street Addresses only. No P.O. Boxes.) To avoid duplicate orders, please be
sure that only one person in your council Is responsible for ordering the 5th Sunday Rosary
program Kit. Each Kit contains the following items:
