This has been a vital service for the senior and disabled residents of our area. Due to inevitable changes, it has become necessary for us to revamp our program. Our service is available for members of the Community Center. Membership is $10.00 per household per year. 1. You must call the Community Center at 878-2702 no later than 1:00 pm the day before and no more than 72 hours in advance to schedule your ride. Ongoing rides will not be accepted. 2. Have the time of your appointment and the address of where you need to go available when you call to set up your ride. 3. If you should need to cancel your ride, call the Center as soon as possible. 4. Your round trip fare authorizes you to make a total of three stops. All medical appointments in Medford must be made between 10:00 a.m. and 3:00 p.m., Monday, Tuesday, Thursday or Friday only. Any deviation to this schedule must be approved by the Director prior to the ride being given. 5. The van will endeavor to leave Medford by 3:30 p.m. This could vary if a medical appointment should run longer than expected. All riders should plan to be ready to leave by this time. 6. Transportation in the Shady Cove/Trail area and from Eagle Point to the Center will be provided on Wednesdays and Fridays. We will continue to provide the Thursday Eagle Point Shuttle from 10:00 a.m. – 2:00 p.m. as demand warrants. There must be at least two riders for the shuttle to run. 7. No early returns from Medford unless there is an emergency as determined by the driver. This causes too much strain on the URCC Van Program and must be avoided. All passengers have options of taxicabs if they want or need a taxi cab type of service. 8. Our area of service is the Upper Rogue area that includes Prospect, Trail, Shady Cove, Butte Falls and Eagle Point and parts of White City. 9. Medical passengers have first priority, disabled next, then senior shoppers. All others ride on a space available basis. 10. Personal Care Assistants will be charged a $2.00 flat fee. 11. All fares are calculated as round trip. No reduction in the fare will be allowed for a oneway ride. As approved by the Board of Directors May 2006 Upper Rogue Community Center Fare Rates Rates are effective as of April 1, 2003 Please note the following round trip fares including rates for non-member passengers. Billing for Medical appointments and trips to the pharmacy is available for individuals covered by Medicaid (OMAP/Medicaid/OHP/AFS) resulting in no fee at the time of service subject to insurance eligibility verification. In compliance with Medicaid rules, we cannot offer discounts or adjustments on rates to medical appointments for those covered by the above-mentioned programs. Rides are available on a first come first serve basis giving individuals 60 years of age or older and disabled passengers priority. Monday, Tuesday, Thursday, and Friday, transportation will be provided to Medford Area. Wednesday and Friday transportation will be provided locally. Please call our office 24-hours in advance for rides to assure a place on the van. All Medicaid (OHP) rides to doctor’s appointments or pharmacies must go through Translink first. You can reach them at 842-2060. You will need to request that the Upper Rogue Community Center provide the transportation.
