Computer Sciences Application Form 2
Name
______________________________________________________________________________________
If the form is not entirely suitable to your background, make whatever adaptations are necessary. Use
abbreviations, comments, footnotes, and additional facsimile pages as needed. Do not mark the form "see
transcript" or the like in lieu of completing it.
COMPUTER SCIENCE COURSES
List in the order taken all the college and university level courses in computer science that you have taken,
are currently taking, or plan to take before entering this department. Include courses according to their
subject matter, not the name of the department offering them. List courses in hardware and circuit design
under "Other Related Courses" on the other side. Include all instances of courses taken more than once.
For "Date" give the semester or quarter and year (e.g., F 99, Sp 00). For "Subject&Number" give the
identifier of the course (e.g., CS 400). If the title of a course is not adequately descriptive, you may add
your own very brief description or comments. Next to or on the line below the title, please give the
author(s) of the course text(s). Under "Grade" enter the grade or mark received as it appears on your
transcript if you have completed the course. Do not enter expected grades or marks. Under "Credit" enter
the credit received or expected. You may add "G" to identify graduate level courses.
