solely as a guide to UW-Extension’s documentary expectations for 4-H Organizations who are
included in UW-Extension’s group tax exemption. This sample agreement may not be
appropriate for all situations where a 4-H Organization is paying an individual for services
rendered.
AGREEMENT FOR PERSONAL SERVICES

6. Liability. To the extent permitted by Wisconsin law, the parties agree to save, hold
harmless and defend each other and their respective agents, employees, and officers
against any and all liability, claims, and costs of whatever kind or nature, for injury to or
death of any person or persons, and for loss or damage to any property or property
interest or loss of personal or financial interests occurring in connection with or in any
way incidental to or arising out of the actions or any activities associated with the terms
of this Agreement.
7. Signatures. The representative of each party executing this agreement, as evidenced
by the signatures below, has the authority to do so.
Please check with your local 4-H Youth Development Educator to ensure that this
agreement is in compliance with all relevant UW-Extension policies and
procedures before it is signed.
