Official Hiring Flow Chart for Adjunct Faculty
Revised, September 2012
This hiring flow chart has been developed through the cooperation of Department Chairs, Associate
Deans, Assistant Deans and Deans at Lawson State Community College. With that in mind, it is the
responsibility of all supervisors and candidates to follow the proscribed hiring procedures, effective
September, 2012,
Step 1: Interview Candidate.
Step 2: Have NEW EMPLOYEE complete and/or secure the following:
1. Application for employment (if not previously submitted)
2. Original Social Security Card and Driver’s License
(For HR Use Only; copies will not be accepted)
3. Request, Authorization, Consent and Release for Background Information
4. Employment Data Form
5. Faculty Credentials Approval Forms
Step 3: Collect completed paperwork from the new employee and STAPLE a copy of the employee’s
unofficial transcripts to the back of the Faculty Credentials Approval forms.
Step 4: Supervisors must perform the following actions:
1. Assess qualifications and complete the Credentials Approval Form
2. Secure appropriate signatures for completion of the bottom half of the credentials form.

Approval is required from the Department Chair, Associate Dean, Academic Dean/Career Technical Dean
and the Vice President of Instruction before processing can begin.
3. Complete the Intent to Hire Form
4. Ensure that the new employee has all items listed in Step 2 before escorting them to Human
Resources. (Incomplete packets or employees without original SSN cards or driver’s
licenses will not be processed in the HR Office)
Step 5: Once the credentials packet is complete, make a copy for your records and escort the new
employee to HR for processing. Please do not bring a new employee to HR if their packet is
incomplete or if they do not have a valid social security card and driver’s license.
Step 6: Submit the original copy of the Hiring Packet to Human Resources for final processing.
Step 7: Once processing is complete, Human Resources will provide the Department Chair with the
employee number and classes may be assigned at this time.
NOTE: Human Resources will handle all Official Transcript Requests. Employees have 14-business
days to submit official transcripts. Failure to render official copies can potentially void the teaching
contract.

FOR BACKGROUND INFORMATION
I have been informed and acknowledged that on December 13, 2007 the State Board of
Education adopted Policy 623.01 requiring criminal background checks for all new and current
employees.
I understand that I may voluntarily consent to the use of my social security account number for
the purpose of conducting a criminal background check. I further understand that my voluntary
consent to use my social security account number is being requested for purposes of
conducting a criminal background check, pursuant to the authority of the State School Board
Policy 623.01.
The information I have given in my employment application, interviews, and/or related resumes
and documents is true, complete, and accurate.
I understand and agree that if employed, and/or during any period of employment, any false
statements, misrepresentations of facts, or omission made by myself become known, my
employment shall be subject to immediate termination.
I understand that in the event a conviction for a felony or any crime involving moral turpitude is
found that the procedures set out in the guidelines for State Board Policy 623.01 will be
followed.
