THE UNIVERSITY OF TEXAS MEDICAL BRANCH AT GALVESTON ISSUED DATE: 07/16/04 LOGISTICS/ACQUISITION OPERATING PROCEDURES REVISED DATE: 06/26/08 --------------------------------------------------------------------------------------------------------------- ---------------------------------SECTION 2 PROCUREMENT SUBJECT 2.57 SECURITY CLEARANCE REQUIREMENTS GENERAL INFORMATION This procedure is issued to incorporate UTMB’s Security Clearance Requirements and Drug Testing in procurement solicitations, Purchase Orders, and Contracts/Agreements. AUDIENCE Applicable to all Procurement Staff and designated Satellite Buyers. GUIDELINES 1. 2. In accordance with UTMB’s Security Clearance Requirements, all suppliers that perform work on UTMB’s premises for a period longer than three (3) days and/or twenty (20) hours per year must conduct a Security Clearance Check and Urine Drug Test on its employees, agents, and/or subcontractors assigned to work on UTMB’s premises. Note: This does not apply to periodic visits by Vendor Sales Representatives, which are governed by UTMB’s Vendor Visitation Policy. 1.1 All suppliers that perform work on UTMB’s premises for a period shorter than three (3) days and/or twenty (20) hours per year must be escorted, at all times, by a UTMB employee. The Buyer will be responsible for including the appropriate Security Clause in their procurement solicitations, Purchase Orders, and Contracts/Agreements, as well as, determining the appropriate Level of Security Clearance. Attachment A to this procedure includes the clauses for procurement solicitations and Purchase Orders/Contracts/Agreements. Attachment A also includes the “Attachment” that should be included with all procurement solicitations, Purchase Orders, and Contracts/Agreements for procurements where the supplier will perform work on UTMB’s premises. Below is a description of the different Security Clearance Levels and the building that are effected at this time. Any questions should be directed to the Compliance Manager. 2.1 Level One Required for access to all UTMB Public facilities. a) Texas Conviction Check b) Terrorist Watch List c ) Sex Offender List 2.2 Level Two Required for all non-escorted work in all Non-Public Areas as listed in 2.2.1 below: a) Criminal History Check for last seven (7) years including residence and employment b) Terrorist Watch List c) Sex Offender List 2.2.1 Non Public Areas M004 Graves Hospital M005 Surgical Research Building M007 Clinical Science Building (5th. Floor only) All Mechanical Areas 2.3 Level 3 Required for all work in Secured areas. In addition to the security clearance requirements identified in 2.2, above Supplier shall have each employee, agent and/or subcontractor's personnel who will be working in a Secured Area contact UTMB’s Office of Environmental Health and Safety at 409-772-1781 to obtain further instructions and additional security clearance. Escort by approved UTMB personnel will be required at all times for all work in Secured Areas. 2.3.1 Secured Areas a) Select Agent Laboratories of M002 Keiller Building Note: UTMB badged Contractor personnel that have Level 2 security clearance may be authorized to work unescorted in non-select agent areas of M002 Keiller Building. Those contract personnel must be escorted by the shift supervisor at entry to Keiller and must card swipe in at the beginning of the shift and card swipe out at the end of the shift. b) The BSL4 Annex c) M002 Loading Dock d) Galveston National Lab EXCEPTIONS There will be no exceptions unless by formal addendum to this procedure or other formal written exception by the Purchasing Agent. 2.57 Security Clearance Requirements Approved: Jeffery Bonnardel, C.P.M.
