This form is required for new course submissions only
All new course proposals require approval from the University Staff
Development Committee (USDC) http://www.uq.edu.au/staffdevelopment/contacts
COURSE RE-SUBMISSIONS - please email course changes, bookings & budget requirements via Excel, Word or write changes
onto the Objectives & Requirements report (available from Training Reports) and return to Staff Development.

Venue Bookings
LIB – Learning Innovation Building no. 17, Seminar room 202 - please enter
your request for LIB 202 Seminar room bookings here & note your preferred date/s and
time/s. Staff Development will try to accommodate your request. Please be mindful that
there is only one training room available to Staff Development for these bookings. It is not
always possible to book the room on the specific dates & times you request.
All other venue bookings are the responsibility of the Course Coordinator. Please
enter details of confirmed venue bookings for venues other than LIB 202 (including
campus) or enter TBA for bookings not yet finalised. When your venue booking is
finalised please email the details to staffdev@uq.edu.au ASAP.

Funding approval from the USDC is required for catering.
Catering will only be provided for approved USDC courses
running for a period of 3 hours or more.
Catering orders need to be emailed to Staff Development
at least 5 working days prior to course commencement.

/ afternoon tea
Equipment
Standard morning & afternoon tea consisting of
tea/coffee & biscuits will be supplied for courses held in the
LIB - Learning Innovation Building #17, Seminar room 202.
Budget allocation for the provision of m/tea & a/tea
provided in this venue will be automatically calculated to
come from the USDC annual budget at the cost of $50 for
a ½ day course & $75 for a full day course.
Equipment hire: e.g. camera, UQ hire car, large projector
screen etc.

Handouts: Standard handout printing requests with instructions need to be emailed to
staffdev@uq.edu.au at least 5 working days prior to course commencement.
If you have approved USDC funding for special handouts which require binding, folders, different
coloured pages etc please email your handouts with printing instructions at least 10 working days prior to
course commencement to staffdev@uq.edu.au.
