Topic: Long-Term Care Options for Older Adults & Who Pays

Target Audience: Physicians, Nurse Practitioner, Physician Assistants, Nurses and
Social Workers in rural Michigan.
Objectives: At the end of this session, participants will be able to:


Identify three long-term care options for older adults.
Identify two payer sources for each of the following services: in-home services, assisted living
and nursing home.
Identify one resource for recognizing quality care for each of the following services: in-home
services, assisted living and nursing home.

Michigan State University is accredited by the Accreditation Council for
Continuing Medical Education to provide continuing medical education for
physicians.
Michigan State University designates this live activity for a maximum of 1 AMA
PRA Category 1 Credit™. Physicians should claim only the credit commensurate
with the extent of their participation in the activity.
Nurses- Participants who attend the entire session and complete an evaluation
form will receive a certificate for 1.0 contact hours.

provider of continuing nursing education by the Ohio Nurses Association (OBN-00191), an accredited approver by the American Nurses Credentialing Center’s
Commission on Accreditation
Certificates: Certificates will be sent to the participant via
e-mail upon successfully filling out and returning the
attendance form and completing the evaluation form online. Physicians can obtain a certificate through the new
CME portal. You will need to create a profile the first time
you use the portal. Session specific information will be
provided to you regarding how to earn and claim your
credit. This information can be found on the session
specific Activity Information Page. If you do not follow the
steps, you will NOT earn CME credit.

Required Forms –Nurses requesting continuing education
must fill out and return the specific nursing attendance
form and complete the evaluation form on-line.
Cost: There is no cost to participate in the
Videoconference presentation. Participants who choose to
listen via teleconference (LIVE audio) may have a long
distance charge.

The Michigan Center for Rural Health is an approved provider with the
Michigan Social Work Continuing Education Collaborative. Approved Provider
