Consent Form for:
A Building Navigator
GGC Students
You are being asked to be a volunteer in a research study
I am being asked to take part in a research study titled “A Building Navigator”, which is being conducted
by Ryan DeMaeyer and Drew Stephenson. I do not have to take part in this study; I can stop taking part
at any time without giving any reason, and without penalty. I can ask to have information related to me
removed from the research records or destroyed.
1: REASON/PURPOSE
This application is used for navigation of the A building on the GGC campus. It will allow users to enter in
a current room number or scan a QR code posted at the hallway intersections. There are three ways the
information will be presented: an augmented reality arrow, a list of directions, or a map with a path
drawn out. Performance of the app will be tested via ease of use and user preference via a pre and post
survey.

3: PROCEDURES
Pre-Survey- This task is used to get some demographic information about you. It will also gauge your skill
level with smart phone use
Tasks: Tasks will be given and observed for accuracy
Post-Survey- This will gauge your satisfaction with the prototype. It will also give you the opportunity to
provide any comments or suggestions
4: DISCOMFORTS, STRESSES or RISKS
No risks are expected.
5: CONFIDENTIALITY
All information concerning me will be kept confidential. My identity will be coded, and all data will be
kept in a secured, limited access location. If information about me is published, it will be written in a
way that I cannot be recognized.
