I.
GENERAL COURSE INFORMATION:
A.
COURSE DESCRIPTION: RNSG 2460 builds on the knowledge and skills acquired in
RNSG courses 1513, 1105, 1144, 1160, and 1115. Taught concurrently with RNSG 1441,
this course is a health related work-based learning
experience that will provide the student with clinical experiences related to the care of patients
and their families in hospital and community based settings. These experiences will enable the
student to apply specialized occupational theory, skills, and concepts. Direct supervision is
provided by the clinical instructor. The selected experiences will develop the student’s ability
to function within the four roles of nursing. Didactic content learned previously and
concurrently in RNSG 1441 will be applied in the development, implementation and evaluation
of care for patients and their families in a variety of settings. Clinical experiences will focus on
common health problems requiring physical and emotional support to patients and their

families experiencing stressors. Increased emphasis is placed on critical thinking/problem
solving and the effects of stressors on patients and their families. Pharmacology, nutrition,
gerontology, growth and development and cultural perspectives will be addressed on weekly
plans of care developed by the student. The student will continue to develop skills necessary to
make appropriate nursing practice decisions by using clinical data and evidence-based practice
outcomes from current literature in conjunction with the nursing process.
RNSG 2460 will present opportunities for the student to practice clinical nursing as applied to
patients and their families seeking to “…maintain or regain balance as the individual moves
along a continuum towards positive competencies” (SPC ADNP Philosophy). The course is
designed to prepare the student to assess,
analyze, plan, intervene and evaluate the nursing care of patients and their families emphasizing
knowledge, judgment, skills, and professional values within a legal/ethical framework.
Through selected clinical experiences the student will learn to function effectively as a Member
of the Health Care Team (one of the roles of a nurse as defined by the Texas Board of Nursing)
by applying the nursing process and principles of teaching/learning appropriate to nursing care
of individual patients and their families. Student grand rounds developed by the student will
contribute to development of teaching skills required to function in the professional world of
nursing. Continuing emphasis is placed on development of skills within the additional three
roles of the nurse, Member of the Profession, Provider of Patient Centered Care, and Patient
Safety Advocate. The student will access appropriate referral sources in order to utilize
resources within the community to meet the various needs of individual patients and their
families.
Students will evaluate their own strengths and needs related to personal and professional
growth and development. This will enhance their ability to make required behavioral
modifications and apply appropriate ethical and legal principles of practice in order to serve
as a health care advocate for patients and their families.

B.
RNSG 2460 SEMESTER STUDENT LEARNING OUTCOMES
Upon satisfactory completion of RNSG 2460, the student will meet the Student
Learning Outcomes (SLO) for Semester II and progress toward meeting the SPC
ADN Program Graduate Outcomes (GO), and Texas BON “Differentiated
Essential Competencies” 2010 (DECS) (see SPC ADNP Nursing Student
Handbook) at the Expected Level of Achievement (ELA) specified for Semester II
of the SPC ADNP. The DECS are listed by letters and numbers in the Clinical
Evaluation Tool on the left side of the page. Roles are designated by the following
numbers:
1 = Member of the Profession; 2 = Provider of Patient-Centered Care; 3 = Patient
Safety Advocate; and 4 = Member of the Health Care Team.
CLINICAL DECISION MAKING
1. Integrate theoretical concepts with nursing knowledge and skills to meet the
basic needs of adult patients in a variety of settings.
2. Analyze evidenced based practice and research supporting clinical reasoning
regarding characteristics, concepts, and processes related to patients, including
anatomy and physiology; pathophysiology; disease processes;
pharmacotherapeutics and other therapies and treatments.
3. Apply steps of the nursing process, which includes comprehensive nursing
assessment of patients, analysis, planning, implementation and evaluation.
COMMUNICATION AND INFORMATION MANAGEMENT
4. Apply technological skills including word-processing, emailing, accessing
search engine databases, bibliographic retrieval and accessing multiple online
resources.
5. Identify regulatory and ethical guidelines necessary to protect confidentiality
when using technology.
6. Demonstrate effective communication with adult patients, families, and
members of the health care team to achieve positive outcomes.
LEADERSHIP
7. Collaborate/delegate effectively with members of the health care team in
planning and decision making to achieve desired outcomes for the adult
patient.
8. Identify the role of the nurse as advocate for patients and their families.

9. Demonstrate clinical decision making that results in finding solutions,
individualizing care, and assuring the delivery of accurate, safe care that
facilitates advancing the patient and support person (s) toward positive
outcomes.
10. Analyze the process for safe medication administration.
11. Identify current National and State standards and guidelines and local
procedures to reduce patient risk.
12. Apply current National and State standards and guidelines and carry out
procedures in a manner that will reduce the patient’s risk of unfavorable
outcomes.

COURSE OUTCOMES: Successful completion of this course requires a grade of
“PASS” for clinical performance and a grade of “C” (77.00) or better in the
concurrent course RNSG 1441. Upon successful completion of this course, each
student will have demonstrated accomplishment of the Semester II Student Learning
Outcomes for the course, through a variety of modes (individual presentations, and
clinical performance).

In order to receive a “PASS” in the course, the student must satisfactorily complete ALL of the
following:

Pass 1 of the 2 required plans of care (CCMs) with a numerical grade of 31
or greater;
Pass the abbreviated plan of care (if assigned);
Pass the additional weekly required clinical preparation work (hospital)
and objectives (off rotations);
Turn in all additional assignments given, in the event of a clinical absence,
by the due date assigned (no exceptions). Note: Failure to turn in the
assignment by the assigned due date will result in an automatic deduction
of 10 points from the final grade of the first scheduled prelim to occur in
RNSG 1441 following the assigned due date;
Satisfactorily pass all additional assignments, given in the event of a
clinical absence, prior to the date that final grades are due for the semester;
Satisfactorily complete all work assigned through the Real EHR Cerner
Prep course;
Show satisfactory achievement of clinical objectives specified on the
clinical evaluation tool;
Show progression towards the Graduate Outcomes at the expected level of
achievement (ELA) for Semester II of the SPC ADNP;
Satisfactorily complete the math competency requirement with a grade that
indicates no more than 2 missed;
Demonstrate satisfactory clinical attendance as specified in Roman
Numeral II Specific Course/Instructor Requirements section F Special
Requirements Clinical Component, of this syllabus and in the SPC ADNP
Nursing Student Handbook.

VERIFICATION OF WORKPLACE COMPETENCIES: External learning
experiences (Clinicals) provide workplace settings in which students apply content
and strategies related to program theory and management of the workflow process.
See Clinical Evaluation Tool for specific competency criteria. Upon successful
completion of the program, students will be eligible to apply to take the state board
exam (NCLEX) for registered nurse licensure.

B.
ATTENDANCE POLICY: The SPC ADNP policy must be followed. Refer to the
SPC ADNP Nursing Student Handbook to review this policy. Also, refer to clinical
requirements specific for this course in the clinical syllabus for RNSG
2460. Allowed number of clinical absences differs from general clinical policy
because there are fewer clinical days. In addition, refer to the attendance policy
found in the South Plains College Catalog.
C.
ASSIGNMENT POLICY:
1. All required work must be in on time in order that the student may benefit from
the corrections and prepare for future assignments. Assigned outside work is
due on the dates specified by the instructor. All clinical correlation maps
(CCM’s) are due on Friday, NO EXCEPTIONS. No CCM will be accepted
or graded after Friday.
If a student picks up a patient on Wednesday and the patient is discharged,
the student will prepare a new plan of care for a newly assigned patient
that will be due Friday.
If your patient is discharged Thursday PM, you may turn in your original plan of
care on Friday. You will be assigned a new patient to take care of on Friday. A
green card will be due on the newly assigned patient by the end of the shift.
2. If you are absent on Thursday, and your patient is still there on Friday, you
will complete your CCM or assigned paper work that you started on Wednesday. It
will be due during Post-conference on Friday. You will still be required to
complete a VCE in the computer lab and complete a CCM, database, lab sheet and
med map for the VCE patient.
If you are absent on Thursday, and your patient is gone on Friday, you will be
assigned a new patient to care for on Friday. You will be sent to the computer lab
to complete a VCE and turn in a CCM, database, lab sheet, and med map for the
VCE patient. You will still need to turn in the original CCM or paperwork
assignment that was due for Thursday morning when you return on Friday. Your
CCM or paper work will not be graded due to the fact that care was not provided
to your original patient.

If you are absent on Friday, your completed CCM or scheduled assignment is due
first thing Monday morning by 8:00 am. NO EXCEPTIONS. This assignment
is considered late. 5 points will automatically be deducted from your total
grade for that CCM. If it is not turned in by 0800 Monday morning, you will
receive a “0” on that map. You will also be required to complete a VCE in the
computer lab, and turn in a CCM, database, lab sheet, and med map for the VCE
patient.
If you are absent on Thursday and Friday, and you never took care of your
assigned patient, the CCM for that week is considered a failure. You are still
required to pass 1 CCMs out of 2 total in order to pass RNSG 2460. You will also
be required to complete 2 VCE assignments in the computer lab, and turn in a
CCM, database, lab sheet, and med map for your primary VCE case assigned.
VCE case studies are required assignments for missed clinical experiences and DO
NOT replace a grade or an absence.
3. Lab prescriptions- a prescription will be assigned by the clinical instructor for any
specific skill that he/she decides needs further practice. The student must perform
the prescribed task in the skills Lab until the learning objectives are safely met
before the skill is performed in the clinical setting.
Lab prescriptions are considered student assignments and documentation of
completion must be returned to the assigning instructor.
4. Students should retain a copy of ALL assignments turned in. If an assignment
turns up missing, the student is responsible for turning in the copy or redoing the
entire assignment if a copy was not retained.
D.
GRADING POLICY: Refer to the SPC ADNP Nursing Student Handbook
Grading System.
1. The math competency exam must be passed with a grade that indicates no
more than 2 missed in order to continue in RNSG 2460 and RNSG 1441. The
student may take the exam up to three times.
2. Weekly nursing plans of care (CCMs) or assi gned prep work will be
reviewed and/or graded by the course instructors. Grading criteria is provided
and available in Blackboard.
3. You must pass 1 out of 2 total CCMs with a score of 31 points (77%).
If you fail the 2nd CCM, this will result in a failure in RNSG 2460,
and you will be removed from the course. If you pass your first CCM,
you will move to an abbreviated version of clinical preparation which is
graded as Pass/Fail.

Please NOTE: Abbreviated does not mean incomplete. You are expected to complete this
assignment thoroughly. If you turn in an incomplete abbreviated CCM, you will be
required to complete a full CCM which will be graded and must be passed with a minimum
grade of 31. If this happens to fall on your last week of clinical, you will be given an
incomplete in the course until your work is turned in and graded. An incomplete means
you will not move forward to the next semester.
NOTE: PASSING 1 CCM DOES NOT GUARANTEE A “PASS” IN
RNSG 2460. You are expected to demonstrate continuous growth and
improvement from week to week. (See Section F: Special Requirements)
4. On the Clinical Evaluation Tool, each student must achieve at least a majority
of satisfactory ratings on individual objectives within each graduate outcome
on the final evaluation in order to attain the expected level of achievement for
Semester II Graduate Outcomes and to pass RNSG 2460.

Required clinical skills must be passed:
Sterile Procedure: Can be a foley insertion, CVC dressing change, sterile
wound dressing change, trach suctioning/trach care or new G-tube site care
Dressing Change: Non-Sterile
Examples of non-sterile dressing: decubitus dressing care, surgical wound
sites over 2 weeks old requiring a dressing.
NOTE: Removing a peripheral IV, removing an On-Q Pump, or removing a
foley catheter DO NOT COUNT as a clean dressing change or sterile
procedure.

5. Important note: A student must obtain a “Pass” on all components
of the clinical course (required plans of care (CCMs), assigned abbreviated
plans of care, required assignments when CCM’s are not scheduled, required
assignments for clinical absences. clinical evaluation tool, required objectives

for off rotations) in order to receive a PASS in RNSG 2460.
E.
DRESS CODE
1. Students must wear the approved South Plains College ADN Program uniform
top and bottom. The uniform must be clean and pressed. An approved tan
colored short-sleeved undershirt may be worn underneath the uniform top or
the SPC dickey. Long sleeved tan undershirts are allowed only when covering
tattoos on the arms. Shoes must be white leather (no canvas or mesh) with
closed toes and closed heels. Appropriate undergarments must be worn
including white socks. An approved tan colored long sleeve lab jacket may be
worn, if needed.
2. Hair must be WASHED and pulled up off of the collar and contained in some
type of knot. Ponytails, feathers, and other hair adornments are not allowed.
All tattoos are to be covered. Only one set of ear studs placed in the ear lobes
are allowed. No ear gauges. No facial or tongue piercings/ear gauges are to
be worn to the hospital, or to any of the other rotation sites.
3. Student badges must be worn at all times in a clinical agency.
4. Nails must be trimmed short with no polish, and acrylic nails are not allowed
at all.
5. Students must pay attention to body odor, cigarette stench, and halitosis (bad
breath) at all times. No cologne or perfumes are allowed. Foul odors are
offensive to patients and staff members.
6.
Students must wear clean and pressed SPC uniform to the hospital on
Wednesdays when picking up patients, and to post conferences on Fridays.

Note: The student must pass the math competency test with a grade that indicates no more than 2
questions missed prior to beginning the clinical rotation and in order to remain enrolled in RNSG
2460 and RNSG 1441 (Three opportunities are provided to pass the math test).
If a student passes RNSG 2460 and fails RNSG 1441, they must retake RNSG 2460 along
with RNSG 1441 because they are co-requisite courses. This is also the case if a student
fails RNSG 2460 and passes RNSG 1441
F.
SPECIAL REQUIREMENTS:
1. Clinical Component:
a.
The clinical course (RNSG 2460) will be graded on a Pass/Fail (P/F)
basis and must be passed along with RNSG 1441. The two courses
must be taken concurrently and failure in one of the courses means you
must repeat both courses.
b.
Clinical is a learning experience that cannot be taken for granted and
absences severely compromise that experience. If you are running a
fever and ill enough not to attend clinical you must notify the
instructor by phone and the unit you are working on. The first
two absences (2 clinical days) will require completion of outside
assignments (VCE) designated by the instructor.
The third absence will result in automatic clinical failure and be
grounds for removal from the course unless there are extenuating
circumstances. The Admissions Academic Standards Committee will
review all cases of extenuating circumstances at the student’s request.
Their decision will be final.

c.
Each student must pass the math competency exam test prior to
beginning the clinical rotation, and must pass the math competency
exam in order to remain enrolled in RNSG 2460 and RNSG 1441.
d.
The clinical instructor may remove the student from the clinical
setting/course if the student demonstrates unsafe clinical performance
as evidenced by any of the following: (This list is not all inclusive)

Places a patient in physical or emotional jeopardy.
Inadequately and/or inaccurately utilizes the nursing
process.
Violates previously mastered principles/learning objectives in
carrying out nursing care skills and/or delegated medical
functions.
Assumes inappropriate independence in action or decisions
which include unprofessional conduct of any kind while in
uniform.
Fails to recognize own limitations, incompetence and/or
delegated medical functions, thereby jeopardizing the patient’s
safety and welfare.
Fails to accept moral and legal responsibility for his/her own
actions, thereby, violating professional integrity as expressed
in the Code for Nurses and the Nursing Practice Act.
Inadequately prepares for the clinical experience as
evidenced by: lack of /or poorly written CCM, data base,
med map, or lab sheet.
Inadequately prepares for the clinical experience as evidenced
by: Failure to follow 5 rights of medication administration
AND/OR failure to discuss medications with instructors
accurately before administration. This is an important safety
issue.
For a positive drug screen from a random or for-cause drug
test. (Refer to the departmental drug testing policy for
information regarding random and for-cause drug testing)
NOTE: Any of the above incidents will result in
clinical failure and/or dismissal from the ADNP.
e.
The SPC Uniform Policy must be followed in the clinical area. See the
ADNP Nursing Student Handbook. YOU MUST WEAR
YOUR BADGE AT ALL TIMES, (NOT A PIECE OF TAPE WITH
YOUR NAME ON IT) OR YOU WILL BE SENT HOME. THIS
MAY COUNT AS A CLINICAL ABSENCE.

Skills Laboratory Component: The skills laboratory component serves the
following purpose:
Provides the opportunity for students to practice their skills and demonstrate proficiency
for required skills prior to clinical performance of those skills. This will minimize
student anxieties in the clinical setting and maximize positive production competencies
for both the patient and the nursing student
III.
COURSE OUTLINE:
There is no specific course outline as this is a clinical course. The student is expected to
be available for clinical assignments and duties on Wednesday, Thursday, and Friday of
each week (see clinical schedule for actual times). Actual hospital clinical days will be on
Thursday and Friday with assignments and preparation time scheduled for Wednesdays.
Off rotation experiences will be assigned on Wednesdays and Thursdays or Thursdays
and Fridays (clinical schedule and explanations available on Blackboard).

Grading criteria for your plan of care is provided for your information.
Please review and compare your care-plans to the criteria prior to
submitting the plan for grading. Please note that points will be deducted for spelling,
grammar and punctuation.
2.
Please see your instructor if you have questions about the grading criteria or a grade
received on a plan of care.
3.
New nursing diagnoses must be used on each CCM for the identified major priority
nursing diagnosis. Nursing diagnoses may not be repeated on CCMs for the major
nursing diagnosis.
4.
NOTE: You may NOT complete a CCM on the same patient twice.
5.
You must pass 1 out of 2 full CCMs OR 1 full CCM AND an abbreviated CCM.
If you fail 2 CCMs, this will result in a failure in RNSG 2460,
and you will be removed from the course at the time of the second failure.
If you pass your first CCM, you will move to an abbreviated version of
clinical preparation for the second CCM. If you fail the abbreviated
version you will be required to complete a full CCM on the failed
abbreviated CCM. If you fail to pass this additional assignment (despite
having passed one CCM) you will fail clinical and will be removed from
RNSG 2460 at that time for failing to continue to show adequate
progression.
NOTE: Passing 1 CCM DOES NOT GUARANTEE A “PASS” IN RNSG 2460.
You are expected to demonstrate continuous growth and improvement from week to
week. (See Section F: Special Requirements)

Additional Clinical Information
TPN / POLICY & PROCEDURES FOLDER
Bring Policy & Procedure folders (including TPN information) to clinical every week. It is
suggested that you review this information and tab important procedures you may be performing
ahead of time.
LISD
Review the LISD assignment schedule. Find your assigned nurse and campus. Call your nurse 2-3
days prior to your rotation to receive directions to school, parking information, etc. Report to your
campus no later than 0745. You will be there from 0745 until 1300 or as directed by your nurse.
Wear your SPC uniform to LISD rotation.
Please take your LISD questions with you AND a copy of your LISD security clearance form.
This rotation will be on Wednesdays. Note: Wednesdays will show as a 2 on your clinical schedule.
You will report to post-conference on Friday in your uniform.
ADULT DAY ROTATIONS
Adult Day Activity Center : Report to the Adult Day Activity Center located on 2807 42nd St. (42nd
and Canton). The exact address is 2807 42nd St. The phone number is 793-1101. See Nikki Oplie
for your schedule for the day. Take your questions for Adult Day with you.
Lubbock Adult Day Center : Report to the Lubbock Adult Day Center located at 3306 83rd St. The
phone number is 796-2273. See Alice for your schedule for the day.
Take your questions for Adult Day with you.
Adult Day Activity Center: You will go to 42nd & Canton (2807 42nd Street) on Thursday.
Lubbock Adult Day Center: You will go to 3306 83RD Street (right off Indiana Ave, behind Pizza
Hut) on Wednesday.
This rotation will be from 9:00 AM to 3:00 PM on Wednesday AND Thursday.

PERIOPERATIVE EXPERIENCE
The perioperative nursing rotation will be completed on Friday of the week you are assigned as a UAP.
(UAP will be on Thursdays). (Consult your clinical schedule to see which unit you will report to).
Consult your clinical schedule for your rotation date. You will report to the OR area of University
Medical Center no later than 6:00 AM if your schedule says 1A. Orientation to this area will be
completed on the day you report to the OR. You will report to Jackie Jackson or Estella Morris. You
will be shown where to go to get dressed.
You will report to the Covenant Surgicenter no later than 6:15 AM if your schedule says 1B. You
will report to Olga Lara or Jennine Swindell. For your surgi-center rotation you are to report the
day before, no later than 4:00 PM to get fitted for your scrubs and pick up your patient case for
the following day.

Take your Perioperative questions with you on the day of your OR experience.
Wear your uniform to this rotation
Be prepared to work, and not just observe. There may be opportunities to start IVs, insert
catheters, etc………you must be with the RN to perform these procedures.
You will report to post-conference on Friday in your uniform.
VIRTUAL CLINICAL EXCURSIONS (VCE)
If you are assigned a VCE your assignment will be emailed to you through Blackboard. You must sign the
contact record specifying your assignment on the first day you are on campus following receipt of the
assignment. This record will be kept in your course file.
Report to the computer lab in Levelland. Bring your textbook, a nursing diagnosis book, your lab book,
and a pharmacology book. You will receive a case study that you are to complete when you arrive.
Allow 4-6 hours to complete this assignment. This assignment will remain in the computer lab
following completion.
You will also complete a CCM (clinical correlation map), a database, a lab data sheet with
interpretations for abnormal values and nursing interventions that explain how the abnormal values can
be improved, and a med map. This portion can be completed in the computer lab or at home. The entire
assignment will be due on the date specified.
The assignment must be turned in no later than the assigned date or 10 points will be deducted from the
final grade of the fnext scheduled prelim in RNSG 1441.
The assignment must be completed prior to the date grades are due for the semester in order to receive a
“P” in RNSG 2460 and progress to the next semester.

MISCELLANEOUS INFORMATION
Full plans of care (CCM’s) will be completed as assigned on your clinical schedule. You will
spend a minimum of one week on each floor rotation.
Students in rotation to LISD, Adult Day, and OR will give an oral report about their rotation in
post conference if time allows.

ASSIGNMENT DUE DATES
Clinical Correlation Maps (CCMs) will always be due during post conferences on
FRIDAY. You will have 10-15 minutes before post conference to complete your
CCM. It should be nearly completed (including all coloring) by Friday morning.
You should only have to add additional Friday assessment and lab information.

allow you to give your undivided attention to your peers as they present student
grand rounds and clinical information during the conference.
OR, LISD, and Adult Day typed assignments will be due the Monday following the
rotations at 9:30am. Turn all of these in to TAMIE!

punctuation. No handwritten assignments will be accepted.
The Ortho packet (CPM quiz, and Polar Care quiz located in the clinical orientation
packet) and summaries for the total hip and total knee videos located on Blackboard
for the Ortho rotation will be due before your Ortho rotation.

CLINICAL POST CONFERENCES
CCM PRESENTATIONS
Each student will be required to present one of their CCMs during a post conference this semester.
The purpose of CCM presentations is to help you learn to report significant information regarding your
patients and the care you provided utilizing the nursing process. You also benefit from visualizing
significant relationships (correlations) between various components of the CCM.
CCM presentations also help your peers learn about other medical conditions, medications, nursing
interventions and teaching, and you benefit from peer and instructor feedback.
Your presentation will include the following:
1.
Review of the pathophysiology of the disease process.
2.
Discussion of the signs and symptoms of the disease process.
3.
Discussion of surgical procedures performed and the reason
For the procedure (if applicable).
4.
Discussion of possible nursing diagnoses including the one you

5.
