Formative Assessment Plan
Directions: Determine an upcoming learning target in which students typically struggle. In your CT, complete the chart. There should be at least
one method to collect evidence for each criteria for success. After completing the chart, create the product to be used to collect evidence of
learning. Under the documenting evidence column, explain how the product will be used to evaluate student learning. After the chart and
product are completed, return to the larger group and record feedback. Create copies of the plan and product. Leave one copy with the group
leader.
