Various objects, such as text and objects can be added (and manipulated) to create this image. Once the objects are positioned within the image, you can use various Photoshop painting tools to blend the different objects with one another so they appear to be a single, seamless image. By hiding all the layers except the background, the Brush tool and the Airbrush option can be used to create the blended effect. (The images within the playing card and specific areas of the type can be distorted using the Blur tool and the Sharpen tool. Once the painting tools have been used, the layers can be displayed and rearranged to achieve the effect you want.
