2016 Capital High School Foundation Scholarship Application
The Capital High School Foundation oversees approximately 40 different scholarships. Submitting one application will allow you to
compete for several different scholarships. Scholarship award criteria may vary. Most are based on financial need. Others are based
on academic performance, athletic achievement, leadership, career goals, club involvement, and/or community service. Scholarship
award amounts range from $500-$2000 and ONE scholarship is available for a FULL TWO YEAR tuition waiver from SPSCC.
Seniors planning to attend a 4 year, 2 year technical, 2 year community college or any accredited program in the United States may
apply.
You must have a FAFSA completed to apply for these scholarships. Attach your Student Aid Report you received from FAFSA.org. If
you do not know how to access this report you may visit: http://www.finaid.org/fafsa/efc.phtml
No handwritten applications will be accepted; computers in the library or Career Center are available for your use. Download
application from the CHS website & type directly into the application.
Information including financial status and transcript information on your application will be shared with members of the selection
committee and with outside community organizations that sponsor specific scholarships. This information will be treated very
respectfully and not go beyond those members for use in selection.
Please complete this application and return a PRINTED copy to the CHS Career Center by
Tuesday, April 26th, no later than 2:30 pm.

the selection committee).
