Dear Parent/Guardian,
Welcome to the 2015-2016 school year! We are very excited to work with your child this year in
health and physical education. Our goal is to provide a safe environment where students will actively
improve their individual physical fitness levels. Actively participating to master the SOL learning
targets is the key to their success.
Please note that a student may be excused from physical activity by a parent or guardian for
no more than one week (3 consecutive classes) per quarter. A parent note must be given to the
physical education teacher. A doctor’s note must be given to the nurse for any period beyond three
classes. The doctor will be requested to complete a checklist of appropriate activities that the student
may take part in while recovering from the illness or injury. This checklist can be found on the
Health/PE department website. If a student cannot participate in a general physical education
program, the student may be assessed for an adapted physical education program to meet the
requirements set by the state. We look forward to a great year. Thank you for your ongoing support!

Use equipment appropriately.
Keep hands and feet to yourself.

Follow the school rules.
Lock your belongings in the locker assigned to you during and after class.
Do not share your locker, combination, or uniform with anyone.
Cell phones or other electronic devices are not permitted in the locker room.
Spray cans (deodorant, cologne, perfume, etc…) and glass containers are not permitted in the
locker room.
6. Teacher permission is required to get a drink, re-enter the locker room, or go to the nurse.
7. Only go into the equipment room, weight room, or bleachers with teacher permission. Only use
equipment with teacher permission.

I Am Ready To Learn
You should be seated quietly in your squad spot facing forward BEFORE the tardy bell rings.
Wear appropriate attire to participate (gray shirt, navy blue shorts, socks, and tennis
shoes). If you do not have the proper attire, you will be given a rental uniform.
All jewelry (except stud earrings) must be taken off before class.
After changing, sit in front of your locker until dismissed.
When the whistle blows you need to stop, sit down, get quiet, and look at the teacher.
I have the read the rules and expectations. I understand that I need to follow the rules in order to have a
safe environment in Physical Education class. I will accept these rules and understand that there are
consequences for failing to follow them.
