ASCWU-BOD Prohibited Activity Exception Request
The Board of Directors Recognizes that there will arise rare occasions where a ASCWU
organization or entity has good reason for wanting to participate in an activity on the
“Prohibited Activities” list.
This Exception Request is an avenue for an organization to partake in a prohibited activity
upon a majority vote by the ASCWU Board of Directors. Please fill out the following
information and submit this form to the ASCWU Board of Directors office located in SURC
236.

will be present, all safety protocol and risk management precautions being taken by your
club to ensure the safety of all individuals involved, and any other information you feel is
important in the nature of the event.
This form is to be turned in to the Vice President for Clubs and Organizations no later than
one week before the date of the Board of Directors Public Meeting that your club wishes to
present your Exception Request. Any forms turned in less than a week in advance will be
moved to the next meeting.
