YES NO 24 Are contingency plans in existence for emergencies YES NO Action Required Auditor Signature: Position: Witnessed By: Position: D:\219514761.doc SI3 NOTES This checklist is one of a series of forms, issued by the Radiation Protection Office, designed to assist Departments in conducting self-inspections with regard to with the Management of Health and Safety at Work Regulations. The series deals with both ionising & non-ionising radiations. SI3 covers the use of x-ray equipment. It may be freely adapted to meet the particular needs of your Department. The lists of departmental radiation workers, radiation areas & x-ray equipment (DRW, DRA & DXR) referred to may be obtained from the URPO. 1 2 3 4 5 6 7 8 9 10 12 14 15 18 Radiation Workers observed in laboratories can be checked against the list, or copies circulated to relevant Lab. Heads/Project Supervisors. URPO should be informed immediately of any unregistered workers. Assessment of individual competency based on working practices/observed conditions in labs. Each laboratory should have a copy of Local Rules available and visible. These are necessary where re-alignment operations are undertakn by University personnel. Check DRA. Inform URPO of changes 19 20 21 22 Signage available via URPO. Non-classified radiation workers may only enter controlled area whilst following a written system of work. This should be displayed prominently in the area. Copies of such systems of work should be sent to URPO. Locked at all times when not occupied access restricted to those directly involved in the work i.e. no spectators (even if they are themselves registered workers). Inform URPO of any changes Illuminated signs or lights should be designed to be fail-safe; situated near tube with duplication where necessary. Where lights alone are used, their meaning should be made clearly understandable by any person having access to the area. Suitable monitors should be readily available, preferably within the radiation area itself. It is not recommended that monitors are “shared” between labs - where this cannot be avoided there should be good (and documented) reason for such arrangements (e.g. very short term work, intermittent use, etc.) D:\219514761.doc 23 24 Any monitors not responding to radiation should be removed immediately from the radiation area. The URPO should be contacted to arrange for repair/replacement temporary ‘bridging’ loan of a monitor may be possible. Contamination Monitors are regarded as essential pieces of safety equipment in Departments using radioactivity - all costs are Departmental ones. If NO, withdraw from use until batteries replaced. Check when last used! If “no” CONTACT URPO. Frequency of monitoring should be related to frequency of use. Further checks should be undertaken after modification servicing or realignment. The extent to which emergency planning requires consideration will vary with Department. As a minimum, departmental personnel should be aware of whom to contact in the event of a radiation emergency. The URPO should be contacted for advice if these requirements are unclear
