IRB#_____ TITLE: CHILD INFORMATION SHEET We are asking you to be in a research study. You don’t have to be in this research study to get treated. If you don’t want to be in the study your doctor will still take care of you. The goal of this study is to see whether the type of match you have with the donor effects how well the transplant works. Your doctor will take blood from you (by a needle, or through your central line before the transplant, and again at 1 month, 3 months, 6 months, and one year after the transplant. This extra blood will be drawn when you are having other blood tests done. We will also get some medical information from your records and hospital chart. The main risk of being in this research is from the extra blood tests. If a needle is used, then there is a risk of pain or bruising, or infection.
