are dying, go to the hospital.

Where do I get proof of MMR or my immunization record?
• Usually your high school, physician’s office, the military, or previous university will
have a copy on file.
What if I can’t find my immunization record?
• You can get an MMR titer (blood test)
• If you have Gold Insurance, this is free
• You can sign a waiver form so you can be here without having the shot
