Issaquah School District 411 Educational Assistant – Discovery Elementary The District supplies this Performance Improvement Checklist when an employee’s performance is determined to be unsatisfactory for any given section of their annual Evaluation. Every effort will be made to provide training and support necessary for the successful completion of this Performance Improvement Checklist. Improvement must occur immediately and must be maintained. The process of performance improvement provides that if management determines at any point during or after this time period that your overall performance is not significantly improving, additional performance deficiencies develop, or performance regresses, disciplinary action will be taken, up to and including termination. A decrease in performance after successfully completing the Performance Improvement Checklist may result in being dismissed without the issuance of another warning or improvement plan.
