NOTIFICATION OF INTENTION TO CONTINUE ALL EMPLOYMENT DUTIES
IN THE EVENT OF AN UMFA STRIKE
I certify that I will continue to perform all of my employment duties in the event of an UMFA
strike in relation to renewal of the collective agreement that expired [x]. I therefore request
continuation of my salary and benefits from the “effective date” stated below. I realize that the
University will act in reliance on the accuracy of this notification.

The effective date should reflect the first day of the strike, if there will be no
interruption in the performance of all employment duties, or the date on which you
returned to work.

Please deliver this form directly to the office of your dean or director as soon as possible. The
form must be received by your dean or director no later than 4:30 pm on the first day of the
strike or the first day you return to work.

In order to create a list of classes that will be continuing, we require a copy of the form by end of
day [x]. Please email copies to:

lisa.halket@umanitoba.ca (Human Resources)
Where members notify you after this date, please email the form as soon as possible.
In order for the information on this form to be reflected in the X direct deposit, this form must
be received in Human Resources by X. If the form is not received by this deadline, pay will be
processed at the earliest possible date, which will be the earlier of the Friday rush cheque run, or
the next regular payroll.
