Dear Parent or Guardian:
Welcome to a new school year! I am excited and looking forward to teaching your son or
daughter about the importance of physical activity and health. Due to the unique nature
of the physical education class, I feel it is necessary to inform you about my expectations
and requirements.
It is with your son’s or daughter’s best interest that both of you clearly understand what is
expected in my physical education classes. Success of the program is dependent upon
parental support in conjunction with open lines of communication between parents and
teachers. Should you have any questions regarding your son’s or daughter’s involvement
in physical education, please do not hesitate to contact me at 480-472-8326.
Please complete and sign the form at the bottom of this letter acknowledging that you and
your son or daughter have both read and understand all of the information included in this
letter. When finished, please detached the signature page and return to me. It is with

Physical Education Attire



The following guidelines for proper Physical Education attire have been
established and must be followed by each student in order to minimize theft
and maximize the student’s ability to effectively participate.
A Fremont P.E. uniform may be purchased at the Fremont Bookstore.
Uniform t-shirts and shorts may be purchased separately.
Recommended attire if unable to purchase a uniform:
 Light colored T-shirt
 Gym shorts
** Write name on both shirts and shorts
 Socks
 Athletic shoes that lace up
 Sweats or warm-up jogging outfit may be worn in colder weather
 All physical education attire must follow dress code
Grading Policy


The physical education grading policy is based on a percentage and point system.
50% activity participation
A = 90% and above
50% dressing out
B = 80-89%
C = 70-79%
Daily points: 10 points possible
D = 60-69%
5 points for dressing out
F = below 60%
5 points for participation
1 point will be deducted from daily points for tardiness
Locker room/ Classroom Guidelines




Students will be considered tardy if they are not in the Physical Education
building before the tardy bell rings.
Food and soda is not allowed in the locker room or during class at any
time. If your child needs to have a snack because of medical reasons,
please make sure the school nurse is aware of your child’s medical needs.
The school nurse will bring this matter to my attention.
Make sure all ipods, cell phones and all electronic devices are locked up
before leaving for class.
Any cell phone, ipod, camera or electronic device used during class or in
the locker room will be confiscated and taken up to administration.

Medical and Parental Excuses



A dated note, signed by the parent or guardian, explaining why a student needs
to be excused from participation in class is required.
If a student is to be excused for more than one week, a physician’s excuse is
required.
Alternative activity work will be administered if the student is unable to
physically participate for longer than a week.
Make-Up Work Policy for Excused Absences and Excused from Class
Participation by Parent or Guardian


Students are responsible for turning in all make-up work to receive credit.
There are two options for make-up work:
1: Do any type of physical activity for 30 minutes for each day absent.
Example: 2 days absent equals one hour of physical activity.
Document the activity and duration of the activity on any sheet of paper.
A parent or guardian signature is required confirming the activity.
2: Read any article regarding physical education topic (ex. healthy eating
habits, sports, athletes, dance, dancers, new exercise concepts, and nutrition)
from a magazine or internet. Write a summary (at least one paragraph in
length) regarding the article.
Written assignments should contain complete sentences, use of correct grammar
and spelling.
** Plagiarism will not be permitted

All make-up work is due before the end of the quarter. If absent during the
final week of the quarter, please contact me to make arrangements to turn in
make-up work.
Locks and Lockers




Students are assigned and responsible for their own lock and locker. To prevent
theft, keep the locker combination a SECRET.
Lost locks should be reported to your teacher immediately. A replacement lock
will be issued and a $4 fee will be administered if the lock is not found within 24
hours. In the meantime, students will keep their belongings by my desk during
class.
I am not responsible for students’ belongings. Students are strongly encouraged
to label all clothing and keep all belongings locked up during class.
Sharing lockers is NOT allowed. Each student is assigned his/her own locker.
Personal Hygiene and Safety


No glass or aerosol containers (i.e. perfume bottles, hair spray containers)
Showers are not available in the locker room, but are available in the pool area
during the swimming unit.
Consequences for Classroom Violations
There is a bank of consequences for each rule violation. The bank includes the
following consequences: warning, reminder of the rule, conference with
teacher, conference with teacher and parent/guardian, detention, Step-Up
(School Discipline Plan) and administration involvement. The frequency
and/or severity of the rule violation will determine the consequence chosen.
Students are expected to treat their teachers and classmates with respect.
Inappropriate behavior will be dealt with according to the guidelines for student
behavior. Detentions may be given in the occurrence of inappropriate behavior,
gum chewing, profanity, misconduct in the locker room and continued failure to
participate or properly dress out.
Curriculum




Students in Fremont’s physical education classes will be given the opportunity to
explore and sample a wide variety of physical fitness activities, team and
individual sports, aquatics, recreational activities and health concepts. The
purpose is to promote lifetime activities.
Fitness days will include the mile run and various lectures on health and fitness.
Reading and writing assignments will be given out to explore health related
issues, health concepts, current information on fitness, wellness and sport and
recreational activities.
Students will be required to keep a portfolio of fitness assessments and class
handouts.
Planner
Students are expected to provide their planner everyday. Planners will be
reviewed for completion.
Parent and Student Portal
Updated grades are planned to be available by the end of the school day.
Completed make up work is planned to be added to the grade by the end of the
school day.
