NATIONAL HONOR SOCIETY Requirements for Induction Westwood High School Students will be selected from grades 10-12 for induction by a faculty committee based on consideration of all of the following requirements. 1. Students must demonstrate scholarship by meeting the following academic requirements: a. Have a 3.500 cumulative grade point average. b. Have been in attendance at Westwood the equivalent of one semester. c. i. 1st semester sophomores must have 4 full credits (8 semesters) in the areas of English, Foreign Language, Mathematics, Science, and Social Studies. ii. 1st semester juniors must have 8 full credits (16 semesters) in the areas of English, Foreign Language, Mathematics, Science, and Social Studies. iii. 1st semester seniors must have 12 full credits (24 semesters) in the areas of English, Foreign Language, Mathematics, Science, and Social Studies. 2. Students must submit sufficient (see instruction page) evidence of service and leadership on the required application form. Students who qualify academically, but do not meet the other criteria as specified on leadership and service, may attach an addendum to this information form stating extenuating circumstances in their life that have limited their contributions in the areas of service and leadership. 3. Students must be evaluated as having good character by the faculty and administration of Westwood High School. Attendance, participation, courtesy, and honesty will be the criterion. NATIONAL HONOR SOCIETY APPLICATION PLEASE READ ALL INSTRUCTIONS CAREFULLY BEFORE COMPLETING! 1. Fill in, print, and cut apart the teacher evaluation forms and give to every one of your current teachers. You may, if desired, give the extra forms to former teachers, counselors, or administrators. 2. Fill out the application form, pages 1-4, print, and turn in a hard copy to Ms. Guy, room 213, by the due date. No late applications will be accepted. No handwritten forms will be accepted. Do not write your name on any part of the application except for page 1. 3. Information should be complete and accurate. You must submit the application in its original format. It is recommended that you use the online form accessed from the school website at www.mpsaz.org/westwood/activities/clubs/nhs Alternatively you may type your responses, cut and paste onto the form, and then copy the forms. If needed, you may arrange access to a school computer through Ms. Guy, room 213. 4. On page 2-3, give examples of your involvement in service and leadership activities in either paragraph form or as a list. See the partial list of examples below. Please do not hesitate to include other activities which you think fulfill the leadership and service requirements. a) Be specific: what did you do, where and when did you do it. Participation is as important as winning. In other words, trying out for the play is as significant as getting the lead. For example: Submitted entries for Westwood’s literary magazine, April 2004 Was unit manager at Wendy’s, Summer 2004 Played violin in Mesa Symphony, 2003-2004 b) Include at least one school activity and one community activity for both leadership and service. Do not include home or church activities (such as babysitting siblings, or being president of a church youth group) unless your home or church performed a service to the community outside your home or church. c) Do not duplicate activities. Enter an activity only once, either on the service or leadership page. If, for example, participation in a beautification project through the Mayor’s Youth Committee could be either a service or a leadership activity, but would be mentioned in only one place. d) List activities from grades 9-12 only.
