International Student and Scholar Services J-1 Exchange Visitor Student Intern Checklist All materials for the J-1 Exchange Visitor Application process must be submitted together by the sponsoring department. The review process of an application cannot commence until all documentation has been received. The following list constitutes a complete application: Completed by the Sponsoring Department J-1 Intern Departmental Request Form (including all required signatures) J-1 Student Intern English Proficiency Form DS-7002 “US Government Document, please complete the following sections ONLY:” o Site of Activity Information, on page 1 o Contract Agreement – Supervisor Signature & Date ONLY, on page 1 o Training/Internship Placement Plan, complete ALL Sections, on page 2 o Originally signed form must be returned to ISSS Copy of Official Departmental Offer Letter (Stipend details & health insurance, if applies) Completed by the Prospective Internship Participant Completed J-1 Student Intern Biographical Form Completed and signed J-1 Exchange Visitor Insurance Requirement Agreement Financial support documentation (if not fully fund by department sponsor) Please contact ISSS with questions regarding the J-1 Exchange Visitor application process. ISSS requires a minimum of two weeks to conduct a thorough application review. Please keep in mind that a new J-1 Exchange Visitor will be required to apply for a J-1 Exchange Visitor visa by visiting a US Consulate. Visa wait times may vary greatly, and it is important to request a realistic program start date. A program start date less than four weeks from the date of application is highly discouraged.
