4-H Horse & Pony Ownership Papers How to fill out and submit paperwork: A. If the 4-H exhibitor owns the horse, and it is registered in the 4-H exhibitor’s name, please submit: 1. A copy of the horse registration paperwork 2. Two color photos (one of the left side, one of the right side) of the horse 3. Assumption of Risk Form B. If the 4-H exhibitor owns the horse, and it is not registered, please submit: 1. 4-H Certificate of Ownership (page 2) 2. Two color photos (one of the left side, one of the right side) of the horse 3. Assumption of Risk Form C. If the 4-H exhibitor does not own the horse, or it is registered in another family member’s name, please submit: 1. 4-H Horse Lease (page 3) 2. Two color photos (one of the left side, one of the right side) of the horse 3. Assumption of Risk Form Digital and film photos are acceptable. Photos may be printed on a personal computer as long as the horse (including coat color) is clearly visible in the photo. Photos may also be emailed to Sue at sbrooker@illinois.edu they can be printed at the Extension office. 4-Hers may submit paperwork for as many horses as they wish, but may only show two horses, plus a third in the weanlings/yearlings class. 4-Hers are encouraged to submit paperwork for any horse they might exhibit, in case one or more of their animals is not able to come to the fair. All paperwork is due to the Extension office no later than May 9. All exhibitors are reminded to complete the Quality Assurance and Ethics Training by June 9 if they have not done so in a previous year. A lease must be submitted if the horse is registered in a parent/guardian or sibling’s name. Illinois 4-H Horse Lease This Agreement is entered into between _______________________ (Owner) and (4-H member) regarding the horse known as ______ . The term of this lease is from to August 31st of the current 4-H year. The express purpose of this lease is for training and exhibition at 4-H functions. Lessee agrees that while the horse is in his/her possession to keep the horse properly housed, fed and groomed and to maintain any veterinary care that is needed. Lessee and his/her parents/guardians assume full responsibility for the conduct and behavior of this animal. Lessee shall also see that any training is done humanely and in the manner of good sportsmanship. If medical care should exceed $ , both parties will meet and discuss the responsibility of further care. Should the horse die of natural causes and be examined by a veterinarian who verifies this fact, all parties will be held harmless. If the horse dies from negligence, the Lessee is held responsible for the full replacement value of the horse. Both parties agree that if the contents of this agreement are not upheld, the horse will be returned immediately to the Lessor. All 4-H members age 8 and older as of September 1, 2015 are invited to participate. There is no county selection process, but exhibitors must register for the show by June 1.
