In addition to the name of the user, please provide the name of the Principal Investigator under whose name the GMM contained use facility is registered. 2. GMO Register No - This number may be referenced on the cover of the GMM consent issued by the Agency in respect of the GMM contained use activity. 5. Where a number of different projects exist on your GMO Register Entry, please reference the project to which you are referring. If necessary use a different form for each project number. 6. Indicate whether your current GMM activity is classified as a Class 1, Class 2, Class 3 or Class 4 GMM activity. 7. Respond ‘Yes’ or ‘No’ to each of the four bullet points. 8. Where new Class 1 GMM project work is being/has been undertaken, a Risk Assessment completed in respect of the new Class 1 GMM must accompany this form. Where new Class 2 / 3 or 4 project work is being planned, a new notification must be submitted to the Agency in advance of starting any new Class 2 – 4 GMM work. Respond ‘Yes’ or ‘No’ to each of the two bullet points. 10. Where the GMMs are being inoculated into a GM animal, simultaneously submit an Annual Report in respect of that GMO (Refer to ‘Annual Reporting Form GMOs (GM Animals)’. Where the GMMs are being inoculated into a non-GM animal, provide details of the decontamination of the animal remains. If the animal remains are being sent off site for purposes of decontamination provide details of the healthcare waste contractor employed.
