Prerequisites:
Internship I students must have completed Practicum (COUN 551) satisfactory, with “S" grade. Internship II students must
have completed Internship I (COUN 552) satisfactory “S" grade
Applications submitted by July 15, 2015 will receive first consideration for available classes. Late applications are not
guaranteed consideration or may need to accept an alternative location or time for your class.
You must complete the online Internship Orientation before Jul 22, 2015 (Click on "Internship Orientation" on the "Clinical
Applications" page). You must provide complete field site information to the Candace Chuyou-Campbell before July 22,
2015. Please follow the guidelines established in the Checklist for Practicum & Internship for Fall 2015. (Click on "Clinical
Forms" in the "Clinical Applications" submenu). Failure to meet deadlines may result in your being dropped from the course.
PLEASE DO NOT WAIT UNTIL THE LAST DAY TO SEND IN YOUR APPLICATION. WE NEED TO HEAR
FROM ALL STUDENTS WHO WANT TO TAKE THIS CLASS AS SOON AS POSSIBLE.
You will be notified of your acceptance to your section via email.
It is IMPORTANT that you provide accurate and complete contact information including a current working email
address. We rely heavily on email communication and it is your responsibility to maintain a current address and to
check your email regularly. Please notify Marilyn Lewis by email of any changes and complete this application only if
you are applying for Fall 2015.
Application must be completed in its entirety to be considered (more than 1 page)

you are unable to do so, you may type your responses in an email and send it to the office. Save the completed
form on your computer or flash drive so you can locate it, and then send it attached to an email to
cchuyoucampbe@leomail.tamuc.edu. Please indicate in the subject line if this is an application for 516, 548,
551, or 552.

IMPORTANT School Counseling Students--If you are following the School Counseling Track, you must comply
with Professional Certification program, also. Make sure you have submitted an application and turned in all the
required documents to their office (this includes a copy of your service record). This step will save you
considerable time and frustrations later as you prepare for comprehensive exams, TExES, and graduation.
