Application Deadline: July 15, 2015
Prerequisites:
Must be fully admitted to the Counseling Degree Program with a degree plan on file AND must have completed COUN 516
and COUN 528 with a minimum “B” grade (COUN 548 for the 60 hr. program).
Applications submitted by July 15, 2015 will receive first consideration for available classes. Late applications cannot be
guaranteed consideration; could be considered for possible alternative locations or times.
You must complete the Practicum Orientation before July 22, 2015 (Click on "Practicum Orientation" on the "Clinical
Applications" page). You must provide complete field site information by email to Candace Chuyou-Campbell before
August 5, 2015. Please follow the guidelines established in the Checklist for Practicum & Internship for Fall 2015. Failure to
meet deadlines may result in you being dropped from the course.
PLEASE DO NOT WAIT UNTIL THE LAST DAY TO SEND IN YOUR APPLICATION. WE NEED TO HEAR
FROM ALL STUDENTS WHO WANT TO TAKE THIS CLASS AS SOON AS POSSIBLE
You will be notified of your acceptance to your section via email.
It is IMPORTANT that you provide accurate and complete contact information including a current working email
address. We rely heavily on email communication and it is your responsibility to maintain a current address and to
check your email regularly. Please notify Marilyn Lewis by email of any changes and complete this application only if
you are applying for Fall 2015.

form on your computer or flash drive so you can locate it, and then send it attached to an email to

IMPORTANT School Counseling Students--If you are following the School Counseling Track, you must comply
with Professional Certification program, also. Make sure you have submitted an application and turned in all
the required documents to their office (this includes a copy of your service record). This will save you
considerable time and frustrations later as you prepare for comprehensive exams, TExES, and graduation.
