Application Deadline: July 15, 2015
Prerequisites:
Requirement for the 60-hr. program only.
COUN 501 (Orientation) must be completed before Advanced Counseling Skills - Minimum "B" grade required.
COUN 510 (Theories) must be completed before Advanced Counseling Skills - Minimum "B" grade required.
COUN 528 (Group) must be completed before Advanced Basic Skills - Minimum "B" grade required.
COUN 516 (Basic Skills) must be completed before Advanced Counseling Skills - Minimum "B" grade required.
Applications submitted by July 15, 2015 will receive first consideration for available classes. Preference is given to students
who are fully admitted into the counseling program. Late applications are not guaranteed consideration.
PLEASE DO NOT WAIT UNTIL THE LAST DAY TO SEND IN YOUR APPLICATION. WE NEED TO HEAR
FROM ALL STUDENTS WHO WANT TO TAKE THIS CLASS AS SOON AS POSSIBLE
You will be notified of your acceptance to section via email.
It is IMPORTANT that you provide accurate and complete contact information including a current working email
address. We rely heavily on email communication and it is your responsibility to maintain a current address and to
check your email regularly. Please notify Marilyn Lewis by email of any changes and complete this application only if
you are applying for Fall 2015.
Application must be completed in its entirety to be considered (more than 1 page)

You must be admitted to the program (degree or school deficiency) and have a plan on file before you
can be permitted into a section of COUN 551 after COUN 516.
IMPORTANT School Counseling Students--If you are following the School Counseling Track, you
must comply with Professional Certification program, also. Make sure you have submitted an
application and turned in all the required documents to their office (which includes a copy of your
service record). This will save you considerable time and frustrations later as you prepare for
comprehensive exams, TExES, and graduation.
