I also acknowledge that this iPad and its accessories are the property of Pepperdine University and is being loaned to me for the duration of the fall 2010 term for as long as I am officially enrolled in course MATH 214. If I choose to leave the University or to drop this course, I will return the iPad to my instructor Timothy Lucas within one business day of dropping the class or leaving the University. I. Damage I acknowledge and agree that I am solely responsible for the iPad upon issuance by the University. If my iPad is damaged in any way, I will notify my professor and Pepperdine Information Technology at (310) 506-4173 immediately. I acknowledge that I am responsible for any lost class time or missing assignments as a result of not having an iPad or having a broken iPad. II. Loss or Theft If this iPad is stolen or lost, I will report the theft or loss iPad to my campus Public Safety office (310) 506-4442 at the time it occurred and provide a police report documenting the theft of the computer. I will also notify my professor and Pepperdine Information Technology at (310) 5064173 of the lost or stolen iPad immediately after I notify Public Safety. III. Returning the iPad and its Accessories Information Technology will be using your iPad again in spring 2011 for a different course. When your iPad is returned to Information Technology at the end of the fall 2010 term, it will be wiped of any personal information, programs, or applications that were used during the term. Any additional applications you choose to purchase for the iPad will be deleted when the iPad is returned to Information Technology. You will not be refunded for any personal or educational purchases. I agree to be billed for the replacement cost of $700.00 for the iPad, the USB charger cable, and outlet plug and/or an additional $42.41 for the iPad hard case and/or $15.21 for the soft sleeve or any portion thereof, if I fail to return the iPad and all accessories in their original condition (excluding ordinary wear and tear and any repairs covered by the warranty) by the following referenced due date: December 10, 2010. Finally, I understand that the University reserves the right to place a hold on my account in registering for future classes or obtaining my transcripts if I fail to return the iPad and iPad accessories or pay the replacement costs. IV. Research Project I understand that I am being issued this iPad in conjunction with a Pepperdine-sponsored research study to understand the effectiveness and impact of learning technology on student learning outcomes. As such, by accepting this iPad, I also agree to comply with the minimal researchrelated requests associated with the course and iPad. A separate informed consent agreement will discuss the research requirement. V. Use of iTunes Gift Card As part of this study, I will be issued a gift card in the amount of $11.98 to purchase applications for use in course MATH 214: Calculus for Business and Economics. Upon receipt of the gift card, I will install the following applications to the iPad through the iTunes store: 1. Graphing Calculator HD (Cost: $1.99) 2. Numbers (Cost: $9.99) I understand that the installation of the applications listed above is required for my participation in both MATH 214 and the iPad study. I acknowledge that this gift card is to be used to purchase only the applications listed above. Any unauthorized use of the gift card is strictly prohibited. Pepperdine University is not responsible for loss, theft, or misuse of issued gift cards. Lost, stolen, or misused gift cards will not be replaced. I am responsible for purchasing the applications listed above in the event of loss, theft, or misuse of the gift card issued to me. VI.
