_____ 1. Determine the appropriate level and prerequisites for the course (these should be

_____ 6. If the course is a prerequisite for an existing course, attach either a Revised Course or
ROA form revising the existing course.
_____ 7. If the course is a requirement, elective, or restricted elective for an existing program,
attach either a Revised Program or ROA form revising the existing program.
_____ 8. If prerequisites for the course are offered by other department(s) complete and attach

_____ 9. If implementation of new course will involve deletions of any course(s), attach ROA
form(s) deleting the course(s).
_____ 10. Check catalog to determine if the course overlaps with any other IC course currently

HU, FA) the proposal must be submitted to the Humanities & Sciences Curriculum Committee for
review and approval of the distribution designation.
