UNANTICIPATED PROBLEM TRACKING TABLE
This table is to be used or modified to track the number of unanticipated
problems (UPs) submitted to the IRB. At the time of continuing review (CR),
investigators are asked (on the CR application form) how many UPs have been
submitted over the approval period (since initial review or last CR). This table is
only a tool to help the investigator and does not require submission to the IRB.
