Faculty SNAP-O Logic Models Template for SOARS
Develop your Plan of Work off line in this Word document and then paste in each
section. This allows you to be more thoughtful with your planning and develop a higher
quality plan that will be easier to report accomplishments toward. Instructions for
getting into SOARS to paste the information are at the end of this template.
Logic Model SNAP-Outreach (SNAP-O) Narrative Summary
Each Logic Model asks for the Program Title and the Work Area:

2. Healthy Families and Homes (Family Intervention)
Describe the staff make-up of your unit that will carry out the 2015 SNAP-O Education
Plan in the beginning of your narrative in this section.
Families and individuals who may qualify for SNAP food benefits during times of
economic hardship will be the target audience.
Education Delivery Planned: (Copy as many of these sentences as you need to
complete a summarized picture of the type and amount of education you plan to
deliver for FFY2015 in this work area.
Goal 1: Disseminate information and assist potentially qualified persons and current
SNAP benefit recipients at a wide array of sites

the SNAP Education grant program.

 Attend ongoing collaboration meetings as scheduled by DHS or Partners for a
Hunger Free Oregon.
 Encourage participation by DHS SNAP staff in monthly Recipient meetings as
needed. Relay SNAP related issues to DHS contact person. Continue
enhancement of method for clients’ feedback to DHS.
 Continue frequent contacts with local county partners and maintain
connections with local DHS offices that provide SNAP benefits.

Delete any of the samples and text above that you will not be using. Format the text as
needed. Copy and Paste what you have prepared above into the “Describe what will be
done and who will be reached” area. Now go to the next section.

Families who qualify for SNAP will be aware of the program and receive assistance with
completing applications and/or following through with re-applying for benefits. Potential
SNAP eligible individuals will learn how to apply for benefits and eliminate barriers to
continuing benefits.
The Potential SNAP eligible individuals who participate in SNAP Outreach will actually
receive benefits as a result of these activities.

boosts local economies.

To create an individual POW inside SOARS:
1. Log on to SOARS using ONID account and password
http://extension.oregonstate.edu/employees/employee-resources/soars
2. Click on Plans of Work under Annual Reporting on the left menu
3. Click on “add a new plan of work”
4. Choose the FCH program area and the 2015 year and click “create a new plan of
work”
