Thank you so much for volunteering your time to serve as a mentor. We
appreciate the time and effort you will spend or have spent with this senior as s/he
works hard to complete this graduation requirement. Since students select their own
projects, it is important that s/he explain the project to you, what s/he needs from you,
and how you will benefit from the experience (we hope). We ask that you fill out this
form to verify the student’s efforts in hours, attitude, behavior, and personal
