DESCRIPTION OF PARTNERS Mission of the Alabama Cooperative Extension System The Alabama Cooperative Extension System, the primary outreach organization for the land-grant mission of Alabama A&M University and Auburn University, delivers educational programs to the people of Alabama without regards to race or national origin. NOW, THEREFORE, it is hereby agreed by and between the Partners as follows: - - - Clearly state the roles and responsibilities each organization or agency will assume to ensure the success of the proposed project. Describe the resources each partner will contribute to the project either through time, in-kind contribution or with the use of grant funds, including but not limited to, office space, project staff, and training. Identify the representatives of the planning and development team who will be responsible for planning, developing, and implementing project activities and describe how they will work together and with project staff. Demonstrate a commitment on the part of all partners to work together to achieve stated project goals and to sustain the project once grant funds are no longer available. 1. The agreement may be terminated when fiscal support for implementation of the program expires. LIMITATIONS TO THE AGREEMENT VIII. PROGRAM EVALUATION - The partners will participate in an annual evaluation of the program partnership, including assessments to measure changes in knowledge, behavior, and conditions; success stories, and reporting of contacts or clients served. IX. EFFECTIVE DATE AND SIGNATURES The agreement becomes binding upon receiving the last requested authorized signature, and shall remain in full effect until one or all parties expressly agrees to terminate this document in writing. X. AUTHORIZED SIGNATURES a. Partners certify that to the best of their knowledge, the information contained in the agreement is correct and complete and that [Partnering Organization and/or Representative] has full authority to form and to enter into this said partnership; and, b.
