Change of Placement (Requires consultation with a School Psychologist AND Special Education Administrator) Placement changes are alterations of a student’s program model. Examples include, but are not limited to: moving from a resource setting to self-contained moving from self-contained to resource moving from a school-based model to homebased (For dismissals, see “Exits”.) The steps for conducting a Placement Change are as follows: 1. Set up IEP meeting with parent. The meeting should be conducted at the student’s home school. 2. Make sure receiving case manager is invited. This individual will notify others at the receiving school. 3. The Agency Representative will be the sending school’s principal or designee. 4. Provide parent with Prior Written Notice. 5. Conduct the meeting and present data to recommend change of model. 6. Re-write IEP.
