SPECIAL NOTE: Addendum #4 information shall supersede the information contained in the
Contract Documents
This Addendum forms a part of the Contract Documents and modifies the original Bidding
Documents dated January 4, 2014, January 20, 2014, and all prior addenda and clarifications.
Acknowledge receipt of this Addendum in the space provided on the Bid Form. Failure to do so may
subject the Bidder to disqualification.
BID PACKAGE #15: 252 Church St. – Olin Library West Entry Door Replacement
Revise Section 00020 – Existing Condition Photos - Photo No. P-26-7 & P-26-8 to read as follows:
DOOR ‘A’ / DOOR ‘B’
1. DOORS: 1 ¾” thick FRP door (field verify existing door thickness):
• Corrim
• Special Lite
• Commercial Door Systems; or
• Approved Equal



Core: Provide a polyurethane foam core.
Reinforcement Blocking: Provide non-swelling polymer blocking for all lockset,
surface mounted hardware and thru-bolted hardware blocking.
Fasteners: provide corrosion resistant fasteners / thru bolts.
BID PACKAGE #17: 31 McConaughy Dr.; Foss Hill 9-10 Connector – Door Replacement Project
There are two blocks of time available to perform this work due to summer program occupancy.
Contractor shall schedule work accordingly and indicate on proposal form.
