TxDOT – SAN ANTONIO DISTRICT CHECKLIST FOR REVIEW OF UTILITY PERMITS Revised August 23, 2001 OK NOT OK 1) Has the utility permit application been signed and submitted by the utility agency? ______________________________________________________________ ______________________________________________________________ 2) Has the utility agency submitted three (3) copies of complete construction plans showing the proposed utility line? ______________________________________________________________ ______________________________________________________________ 3) Is the location and identification (highway number) of the TxDOT highway clearly indicated on the plans? ______________________________________________________________ ______________________________________________________________ 4) Are the utility plans legible and drawn to scale (or accurately dimensioned)? ______________________________________________________________ ______________________________________________________________ 5) Is the location of the proposed utility line clearly shown on the plans? ______________________________________________________________ ______________________________________________________________ 6) Are the right of way line and edge of highway pavement clearly shown on plans? ______________________________________________________________ ______________________________________________________________ 7) For lines to be installed parallel to the highway, is the assignment from the right of way line (or from edge of highway pavement) clearly shown? For highway crossings, is the location of the crossing clearly shown on the plans? For highway crossings, the crossing should be as close to 90 degrees as practical. ______________________________________________________________ ______________________________________________________________ 8) Does the utility line change assignment unnecessarily? If assignment changes, are the changes justified and reasonable? ______________________________________________________________ ______________________________________________________________ 9) For lines crossing the highway, crossing intersecting streets/county roads, or passing thru the “protected root area” of “desirable trees” is it clearly shown that the line will be installed by boring? It is the SAT District goal that utility lines be located within 10 to 15 feet from the ROW. Utility lines proposed beyond this area will be subject to denial unless adequate justification is submitted and approved. ______________________________________________________________ ______________________________________________________________ 12) Does the utility line installation meet all requirements of the Utility Accommodation Policy (UAP)? ______________________________________________________________ ______________________________________________________________ 13) Is an Exception to the UAP required? If so, has adequate justification been submitted? In general, Exceptions to the UAP must be forwarded to the Austin Maintenance Division or Right of Way Division for approval; and Exceptions are rarely approved. An Exception to the UAP may be required if any of the following applies: (a) Installation parallel to a controlled access highway and located within the control of access area. This type of exception will be denied based on policy from TxDOT Headquarters. (b) Bore pits located within the control of access line of a controlled access highway. This type of exception to the UAP will be denied based on policy from TxDOT Headquarters. (c) Installation by open cutting paved highway travel lanes or shoulders, parallel or crossing the highway. (d) Installation parallel to the highway under paved travel lanes or shoulder. (e) Utility line to remain in place (parallel to the highway) where it will be located under highway pavement due to a currently proposed highway construction, reconstruction, or widening project. (f) Installation (or to remain in place) at less than the minimum depth of cover. (g) Installation for crossing highway pavement or intersecting city streets/county roads without a casing pipe. (h) Attachment of a utility line to a highway bridge. (i) Utility poles located more than 3 feet inside the right of way line. (j) Above ground utility appurtenances that exceed size limits in the UAP. Utility lines should be at least 10 feet from the edge of pavement in urban areas, and outside of the front slope in rural areas if possible. ______________________________________________________________ ______________________________________________________________ 15) Is there adequate space available for this new utility line? ______________________________________________________________ 16) If the utility permit application or plans specifies the minimum depth of cover, does it meet the requirements of the San Antonio District Minimum Depth of Cover Chart? ______________________________________________________________ ______________________________________________________________ 17) Are appropriate temporary erosion control devices (ie, rock berms, silt fences, etc.) shown where the line will be across/along a creek, drainageway, steep slope, within the Edwards Aquifer Recharge Zone, or in other critical areas? ______________________________________________________________ ______________________________________________________________ 18) If the line is to be aerial, will it be placed on existing poles or new poles?
