2. The membership of students in SOA will be lapsed once they have left the SOA Schools User
Unit.
3. It will not be possible to process your application if you fail to provide all the information
requested.
4. The information provided will be used for the processing of HKAYP membership profile
purposes. It may be disclosed to officers in charge of HKAYP.
5. SOA may use the log books / photos of participants for promotion.
6. Applicants have the right to request access to or correction of personal data provided on this
form in accordance with the provisions of the Personal Data (Privacy) Ordinance. Such
requests may be made in writing to this office.
7. This office will send a written notification to schools for buying the Record Books from the
Headquarters of the HKAYP once the application is assessed.
8. Please make copies of the form for your own record.
