K-12 Teacher Mentor Feedback Template:
As a tool to provide your fellow with some mid-year feedback, please write a few sentences
for each of the items listed below. Then, please find some time to discuss your comments
with your fellow. Your graduate student will provide post your comments electronically
through in his/her ePortfolio. These comments will be reviewed for programmatic
evaluations.
1. Pick one of the inquiry-based lessons that your fellow has created. Briefly describe
the lesson. How successful was the lesson? Were the students engaged? Did it allow
students to increase/expand their STEM knowledge and skills?
2. In what ways has your fellow changed in his/her appreciation of K12 education and
in his/her understanding of its challenges, rewards, and special needs?
3. How has the communication abilities of your fellow changed since the beginning of
the year? In particular, in what ways has your fellow been effective in communicating
his/her expertise with students?
4. In what ways has your fellow been successful as a role model of a
mathematician/scientist to your students?
5. Please provide at least one suggestion for ways the fellow could improve the
planning and/or execution of their role in your classroom.
