I, the undersigned, give my consent to release my grades and/or transcript from Texas Woman’s
University to ________________________________________ (Greek fraternity/sorority) and the Center
for Student Development for the purpose of verifying “good standing”, academic progress and awards
eligibility within the organization. This form will be considered active until I graduate, transfer to another
institution, or until I am no longer affiliated with a Greek organization at Texas Woman’s University.

Disclosure of your student identification number is requested as part of your membership in a Texas Woman’s
University Greek-lettered organization. Student identification numbers are maintained and used by the Center for
Student Development for GPA verification and good standing only. Disclosure of your student identification number
and rights will be governed by the Public Information Act (Chapter 552 of the Texas Government Code) and
regulated by FERPA.
