This form constitutes the final step in the CMS Project Management Process. The data gathered will likely be
more anecdotal than scientific but the overall aim is to gather usable information to use in improving our
processes.
Both the Project Requestor and the Project Lead should work together to fill out this form. Once complete, it
should be submitted to the Senior Director, CMS and the CMS Special Projects Coordinator for review.
