Note: Use this application for an individual teacher.

The DIMACS Bio-Math Connect Institute program, running July 17 to July 29, 2005, will be held at

Rutgers University in Piscataway, New Jersey. Participants must have e-mail access from May 2005 through August 2006.

Checklist:

1. Application form

2. Statement of Understanding: signed by applicant and school principal.

3. Graduate Credit Requirements: signed by applicant.

4. Two Letters of Recommendation. One from the school principal and one from the curriculum

supervisor or department chair.

Although these items may be mailed separately, only complete application files will be reviewed.

Deadline for submitting all documents is April 1, 2005.

Early Acceptance deadline is January 24, 2005 .
